ORIGINAL ARTICLE PURPOSEWe aimed to assess the correlation between pulmonary hemorrhage and pneumothorax in computed tomography (CT)-guided transthoracic fine needle aspiration (TTFNA), particularly its possible value as protection against the development of pneumotorax. MATERIALS AND METHODSWe reviewed the CT images of 538 patients (364 males and 174 females, mean age 70 years, range 36-90 years) who underwent CT-guided TTFNA of pulmonary nodules between January 2008 and September 2013. The following CT findings were assessed: pulmonary hemorrhage (type 1, along the needle track; type 2, perilesional; low-grade, ≤6 mm; high-grade, >6 mm), pneumothorax, distance between the target nodule and the pleural surface, and emphysema. RESULTSPneumothorax occurred in 154 cases (28.6%) and pulmonary hemorrhage occurred in 144 cases (26.8%). The incidence of pneumothorax was lower in patients showing type 1 and high-grade pulmonary hemorrhage pattern. The incidence of pneumothorax in biopsies ≥30 mm from pleural surface was 26% (12/46) in cases showing this pattern, while it was 71.4% (30/42) when this pattern was not seen. Similarly, the incidence of pneumothorax in biopsies <30 mm from the pleural surface was 0% (0/28) in cases showing this hemorrhage pattern, while it was 19% (76/394) when this pattern was not seen. CONCLUSION Pulmonary hemorrhage during TTFNA is a frequent event that protects against pneumothorax. A bleeding greater than 6 mm along the needle track is associated with lower incidence of pneumothorax, especially in biopsies deeper than 3 cm.

INTRODUCTIONLung cancer is the most commonly diagnosed malignancy across the globe, especially in males. It is the leading cause of cancer related deaths with an average 5 year survival rate of 16.8%.1 CT guided fine needle aspiration cytology (FNAC) and Biopsy are simple methods to diagnose lung cancer in a vast majority of patients, especially in peripherally situated lesions. FNAC was first used by Martin and Ellis as a diagnostic tool.2 FNAC can sub classify the type of bronchogenic carcinoma and the vast majority of lung malignancies can be confidently diagnosed with cyto-morphological characterization in right clinical context. 3 The purpose of our study was to evaluate the diagnostic utility of CT guided FNAC and to compare it with biopsy in diagnosis of lung cancer. METHODSThis is a cross sectional study, done in department of respiratory medicine, Geetanjali Medical College and Hospital , Udaipur, Rajasthan over a period of 12 months from august 2014 to July 2015. Study was approved by Institutional ethical committee. All the patients with suspected mass lesion on chest X-ray and clinical symptoms suggestive of lung cancer were included in the study. Written consent was taken from each patient. Clinical history was taken in detail and routine blood investigations were sent, including BT, CT and INR. ABSTRACTBackground: Lung cancer is the major cause of cancer related deaths all over the world. CT guided FNAC and/or Biopsy of lung mass is an effective modality to diagnose lung cancer, especially in peripherally situated lesions. This study was planned to compare the role of CT guided lung FNAC and Biopsy in diagnosis of Lung cancer. Methods: This is a cross sectional study, done in department of respiratory medicine of a tertiary care teaching hospital of Rajasthan over a period one year. All the patients with suspected mass lesion on chest X-ray and clinical symptoms suggestive of lung cancer were included in the study. CT guided FNAC and biopsy were done in each patient after explaining the risks. Results: CT guided procedures were done in 81 patients of suspected lung cancer. CT guided FNAC yielded positive results in 75 patients while it was negative in 6 patients. CT guided lung biopsy was positive in all patients. Squamous cell carcinoma was most common type encountered malignancy. Conclusions: CT guided lung FNAC and Biopsy is a simple and safe method with high diagnostic accuracy for diagnosis of Lung cancer.

INTRODUCTIONLung carcinomas are a group of tumors that arise from any part of trachea, bronchi, bronchioles or the pulmonary parenchyma and can be derived from the cells of epithelium, mesenchyme and neural origin. Thus, it is a heterogenous entity both in the variety of cell type and in the biological behaviour of any particular cell type. Bronchogenic carcinoma is the most common cause of cancer-related death in men and second most common in women, worldwide. Imaging studies play a key role in the detection, staging and post-treatment follow up of patients with bronchogenic carcinoma. The strongest prognostic factor for survival in lung cancer cases is respectability of the tumor. Thus, accurate staging of the bronchogenic carcinoma is essential as the choice of treatment options ABSTRACT Background: Lung cancer is the most common cancer worldwide. Hence, timely diagnosis and accurate staging of the carcinoma is critical for the treatment and prognosis. So, this study was performed to evaluate the role of CT scan in diagnosis and staging of lung cancer. Methods: This was a prospective and observational study carried out over one and half years at a tertiary care hospital. The patients found to have abnormalities on chest X-ray suggestive of carcinoma were included in the study. Pre and post contrast CT scans were performed. Individual patient biopsies were done for histopathological staging. Results: Most patients belonged to 41-50 years age group with male preponderance (81.33%). Habit of smoking was prevalent among the lung cancer patients. Almost all masses (92%) showed heterogeneous contrast enhancement on CT. Additionally, there were calcifications, cavitation, hilar enlargement, pleural invasion, mediastinal lymphadenopathy and contiguous bone involvement. Adenocarcinoma was the most common (46.66%) form of cancer followed by squamous cell carcinoma (42.66%). For histopathological diagnosis, majority of patients (73.33%) were diagnosed using CT guided biopsy. In the remaining patients, USG guided biopsy or fibreoptic bronchoscopy were performed. The most common lobe affected by bronchogenic carcinoma was right upper lobe. The most common site of metastasis was pleura (22.66%) followed by bone (17.33%). Majority of the patients (41.33%) presented with TNM stage IV. Conclusions: CT scan as a modality for detection and staging of bronchogenic carcinoma is superior to chest radiograph. CT guided FNAC has a high success rate in evaluation of lung carcinoma.
