Most irreversible blindness observed with glaucoma and retina-related ocular diseases, including age-related macular degeneration and diabetic retinopathy, have their origin in the posterior segment of the eye, making their physiopathology both complex and interconnected. In addition to the age factor, these diseases share the same mechanism disorder based essentially on oxidative stress. In this context, the imbalance between the production of reactive oxygen species (ROS) mainly by mitochondria and their elimination by protective mechanisms leads to chronic inflammation. Oxidative stress and inflammation share a close pathophysiological process, appearing simultaneously and suggesting a relationship between both mechanisms. The biochemical end point of these two biological alarming systems is the release of different biomarkers that can be used in the diagnosis. Furthermore, oxidative stress, initiating in the vulnerable tissue of the posterior segment, is closely related to mitochondrial dysfunction, apoptosis, autophagy dysfunction, and inflammation, which are involved in each disease progression. In this review, we have analyzed (1) the oxidative stress and inflammatory processes in the back of the eye, (2) the importance of biomarkers, detected in systemic or ocular fluids, for the diagnosis of eye diseases based on recent studies, and (3) the treatment of posterior ocular diseases, based on long-term clinical studies.

The impact of rehabilitation measures on the geriatric profile of patients with glaucoma

Background: The high prevalence of glaucoma is accompanied by a steady decline of functional activity due to a significant visual deficit and deterioration of the geriatric status of patients, which is currently insufficiently studied. In the presence of visual deficiency, specialists mainly correct it by surgical methods, but rehabilitation measures aimed at improving the geriatric profile among senile and elderly people are rarely carried out. The aim of the study: To analyze the effect of rehabilitation measures on the geriatric profile of patients with glaucoma. Materials and methods: The study involved 165 elderly and senile patients with glaucoma (the main group), among whom, after laser trabeculoplasty, rehabilitation measures with a geriatric orientation were carried out: physical exercises to improve eye coordination and hand movements; performing cognitive gymnastics; strength exercises; increasing social communications; increasing vegetable protein in the diet, the use of nutraceuticals "Frezubin". In the control group (108 patients), only surgical treatment of glaucoma was performed. A year after rehabilitation, a comprehensive geriatric assessment was performed with the identification of leading clinical deficiency syndromes. Results: Among senile and elderly patients with glaucoma of the main group, at the time of completion of observations, the prevalence of fall risk syndrome significantly decreased from 49.1±4.0 to 22.8±1.9 cases per 100 patients (p<0.01) with a significant difference compared to the control group. Among the patients of the main group, the prevalence of the syndrome of falls and cognitive impairments also decreased. In the control group of patients with glaucoma, only the prevalence of falls syndrome decreased.
