Background: Overactive bladder (OAB) syndrome affects 10–15% of women, severely impacting their quality of life. First-line treatments include behavioural and physical therapy, and second-line medical treatments include medications such as vaginal oestrogen, anticholinergic medications, and ß3-adrenergic agonists—with potential adverse side effects including dizziness, constipation, and delirium, particularly affecting elderly populations. Third-line treatments include more invasive measures, including intradetrusor botulinum injections or sacral nerve modulation, with percutaneous tibial nerve stimulation (PTNS) being a potential alternative treatment. Aims: The aim of this study was to explore the long-term efficacy of PTNS treatment for OAB in an Australian cohort. Materials and Methods: This is a prospective cohort study. Patients underwent Phase 1 treatment, whereby women received PTNS treatment once per week for 12 weeks. Following Phase 1, women entered Phase 2, whereby they received 12 PTNS treatments over 6 months. Their response to treatment was measured by obtaining data before and after each phase using ICIQ-OAB and the Australian Pelvic Floor Questionnaire (APFQ). Results: Phase 1 included 166 women, with 51 completing Phase 2. There was a statistically significant reduction in urinary urgency (29.8%), nocturia (29.8%), incontinence (31.0%), and frequency (33.8%) compared to the baseline. Patients who completed Phase 2 also showed a statistically significant reduction in urinary frequency (56.5%). Conclusions: Overall, the results from this study are positive and support that PTNS is a minimally invasive, non-surgical, non-hormonal, and effective treatment for OAB. These results suggest that PTNS may be a second-line treatment for patients with OAB not responding to conservative management or for patients aiming to avoid surgical approaches.

Background and aim. Despite overactive bladder (OAB) being a symptomatic diagnosis, all patients require a basic assessment to confirm the diagnosis, as well as to exclude any other underlying cause for lower urinary tract dysfunction. While guidelines clearly define different methods of investigations for OAB, there are reasons to believe that these guidelines reflect more of a “one size fits all” model that may not be appropriate for use in all patients. The study aimed to elucidate the complex and advanced multidisciplinary methods of investigation of OAB patients. Methods. PubMed, ScienceDirect, Cochrane Library, and Google Scholar databases were used to search for scientific publications from 2013 to 2023 using relevant keywords and phrases. Results. Data from 708 publications were researched and synthesized, a systematic review was carried out and the detailed and coherent results presented the diagnostic aspects of overactive bladder in women. Relevant citations from selected articles were included, and a synthesis of key information was provided. Conclusion. These discoveries may assist in detecting OAB earlier, allowing for prompt intervention and potentially preventing the condition from progressing to more severe stages and providing healthcare professionals with valuable tools to enhance the diagnosis and treatment of OAB, potentially improving the quality of life for patients affected by this condition.

Neurogenic disorders of the bladder are diverse in their mechanisms and difficult to treat. Researchers around the world have step by step discovered new aspects of the pathogenetic mechanisms responsible for detrusor dysfunction. Naturally, with the understanding of these mechanisms, new lines of therapy appeared, making the treatment of such children more effective and justified. Only complex therapy, considering all pathogenetic mechanisms, can give the best result. To date, the degree of influence and ways to solve the problem of impaired blood supply to the bladder have not been fully studied. For a long time, this issue has not been given due attention, due to the complexity of an objective assessment of the sufficiency of blood supply to the bladder. However, those few studies suggest that it is impossible to count on success in the treatment of such children without including in therapy components aimed at normalizing the angiological factor and, as a consequence, correcting detrusor hypoxia and energy metabolism disorders. Angiological disorders have a multidirectional character from vasospasm and circulatory hypoxia to vasodilation and congestive hyperemia. Therefore, the diagnosis and treatment of blood supply disorders of the bladder is a very important and difficult task. Since neurogenic disorders of the bladder are very common, a universal method of assessing the condition of its vessels is needed, which will be convenient and accessible to doctors in routine practice. Such a method is rheopelviography. The introduction of this method into routine practice will lead to the accumulation of experience and improve the diagnosis and treatment of bladder dysfunctions.
