Diagnosis and Management of Aneurysms Involving the Superior Mesenteric Artery and Its Branches

Abstract: Aneurysms of the superior mesenteric artery (SMA) are an uncommon but lethal entity, which must be treated expeditiously to avoid mortality and high incidence of ischemic small bowel complications. In the past 7 years the authors have treated 4 patients with a variety of types of aneurysms involving the SMA and its branches at a university-based teaching hospital. The first was a mycotic SMA aneurysm as a result of septic mitral valve, the second a jejunal aneurysm in a patient with pancreatitis, the third a s… Show more

“…Rupture is the most feared complication of VAAs and occurs in 22%-24% of patients. 9 About 20% of ruptured smaller mesenteric branch artery aneurysms, as in our patient, do prove fatal. 3 Blood extravasated from the VAA site results in either hemoperitoneum or gastrointestinal bleeding, depending on whether the laceration opens into the peritoneal cavity or into the bowel lumen.…”

“…9 Fever may be a prominent finding in mycotic aneurysms. 17 The natural history of SMA aneurysm appears to be one of expansion and eventual rupture. 18 Other complications of SMA aneurysms include thrombosis and distal embolization causing intestinal ischemia, and erosion into adjacent bowel with gastrointestinal bleeding.…”
