Spontaneous hemoperitoneum from a ruptured mesenteric branch arterial aneurysm: Report of a case

Abstract: We report a case of spontaneous intraperitoneal hemorrhage from a ruptured mesenteric branch artery aneurysm in a patient presenting with syncope. A 54-year-old woman was brought to our emergency department as a medical code, following two syncopal episodes. Computed tomography, carried out to rule out aortic aneurysmal disease, revealed hemoperitoneum without evidence of solid organ injury. “…Major etiologies of JA aneurysm are atherosclerosis, fibrodysplasia, segmental arterial mediolysis, connective tissue disorders, trauma, inflammatory causes, and infections . The elderly age of the patient, the history of smoking and chronic hypertension, and the concomitant presence of AAA have suggested that atherosclerosis has been the etiology of the saccular aneurysm of the third JA in our case.…”

“…Helical Computerized Tomography is particularly useful as it can detect smaller vascular lesions. 14 Angiography has been suggested as the gold standard for diagnosing and localizing these lesions, allowing an evaluation of collateral blood flow in case of obstruction of major splachnic arteries, 6,8,14 providing helpful information in the decision for bowel resection, and facilitating surgical exploration. 4 Intraoperative angiography has also been suggested by McNamara and Griska.…”

“…[1] Since it is characterized by nonspecific clinical manifestations, aneurysm in this uncommon location is usually diagnosed following complications, including rupture, some of which result in death. This report describes the case of a middle aged man with a ruptured superior mesenteric artery (SMA) branch aneurysm.…”
