De novo stress urinary incontinence (SUI) may occur in up to 80% of clinically continent women following genitourinary prolapse surgery. This had resulted in an increase in the rate of concurrent continence surgery during prolapse repair from 38% in 2001 to 47% in 2009 in the United States. To date, there is no local data available to estimate the prevalence of occult SUI (OSUI) among Malaysian women awaiting surgery. Therefore, this study was conducted to elicit the prevalence of occult SUI and its associated risks factors in patients awaiting prolapse surgery. We retrospectively studied the records of 296 consecutive women with significant pelvic organ prolapse awaiting reconstructive repair. All patients attended the Urogynaecology Unit in Hospital Kuala Lumpur Malaysia between October 2007 and September 2011. They had undergone standardized interviews, clinical examinations and urodynamic studies. During the urodynamic testings, all prolapses were reduced using ring pessaries to elicit OSUI. Primary outcome was the prevalence of OSUI with prolapse reduction to predict possibility of developing de novo SUI following prolapse surgery. Secondary outcome was the assessment of potential risk factors for OSUI. Among the 296 women studied, 121 (40.9%) were found to have OSUI. The risk factors associated with OSUI included age, BMI, numbers of SVD, recurrent UTI, reduction of urinary flow symptoms and grade 2 to 4 central compartment prolapses. We concluded that preoperative urodynamic testing with reduction of prolapse is useful to identify women with OSUI. This is important for preoperative counselling as well as planning for one step approach of prophylactic concomitant anti-incontinence procedures during prolapse surgery in order to avoid postoperative de novo SUI.

Currently, minimally invasive surgeries, which are often characterized by reliable and successful results, are preferred for the treatment of stress urinary incontinence. Although all of the currently used surgeries are minimally invasive, morbidities, including hemorrhage, voiding dysfunction, infection, pain, skin infection and erosion, and bladder injuries, are observed. We detected bladder injury in a 42-year-old female patient with complaints of burning and pain during urination who had previously undergone transobturator tape (TOT) surgery. Complete abdominal hysterectomy for a secondary myoma and a TOT procedure had been simultaneously performed 3 months prior to her presentation. Cystoscopy demonstrated a foreign body compatible with sling material in the bladder which was extracted transvaginally.

Пероральные формы препаратов для заместительной гормональной терапии негативно воздействуют на тече-ние стрессового недержания мочи и увеличивают риск его развития de novo. Вагинальные формы эстрогенов оказы-вают терапевтический эффект на все симптомы гиперак-тивного мочевого пузыря (ГМП), включая ургентное не-держание мочи.Менопауза -универсальный физиологический про-цесс, связанный с падением секреции овариальных гор-монов (эстрогены, прогестерон). Диагноз «менопауза» правомочен в отсутствие менструации в течение 12 мес у женщин в возрасте 47-51 года [1]. Время наступления ме-нопаузы зависит от многих факторов, таких как социаль-ный статус женщины, характер питания, образ жизни (ку-рение) и масса тела. Помимо климактерического синдро-ма, характеризующегося вазомоторными нарушениями, падение уровня половых стероидов служит причиной воз-никновения урогенитальных расстройств, включающих различные виды недержания мочи.Урогенитальные расстройства -комплекс вагиналь-ных и мочевых симптомов, связанных с развитием атро-фических и дистрофических процессов в эстрогензависи-мых тканях и структурах нижней трети мочеполового В статье представлен обзор литературы, отражающий современные возможности лечения женщин с гиперактивным моче-вым пузырем в зависимости от возраста. Показана чувствительность нижних отделов урогенитального тракта к половым стероидам, значимость которых в генезе симптомов гиперактивного мочевого пузыря не вызывает сомнений. Рассмо-трены вопросы проведения различных видов заместительной гормональной терапии в состав комплексной терапии.Авторы информируют об отсутствии конфликта интересов. It shows the susceptibility of the lower urogenital tract to sex steroids, the significance of which in the genesis of overactive bladder is beyond question. Different hormone replacement therapy options as part of combination therapy are considered. The authors declare no conflicts of interest.
