Diagnosis and management of acute heart failure

Abstract: Acute heart failure (AHF) is a life threatening clinical syndrome with a progressively increasing incidence in general population. Turkey is a country with a high cardiovascular mortality and recent national statistics show that the population structure has turned to an ‘aged’ population. As a consequence, AHF has become one of the main reasons of admission to cardiology clinics. This consensus report summarizes clinical and prognostic classification of AHF, its worldwide and national epidemiology, diagnostic … Show more

“…The remaining 111 patients were divided into two groups according to age, the VE group (age ≥85 years, n=45) and the not very elderly (NVE) group (age <85 years, n=66). ADHF was diagnosed according to the Framingham criteria as showing at least two of the following clinical signs: dyspnea, orthopnea, rales and a third heart sound, peripheral edema, pulmonary congestion, or jugular vein dilatation ( 13 , 16 , 17 ). The conventional primary therapy for ADHF included some diuretics, inhibitors of renin–angiotensin system (RAS inhibitors), vasodilators, beta-blockers, and noninvasive positive pressure ventilation.…”

“…HF with preserved EF refers to patients with an EF > 50%, while HF with reduced EF refers to patients with an EF < 40%. Borderline preserved EF is defined by HF with an EF of 41–50% 3,4,17,18,29. The most common form is HF with reduced EF, which is primarily related to a decrease in the functional myocardium (typically associated with ischemic disease or a prior myocardial infarction) 3,4,34.…”

“…HF is more common in males until the age of 65, at which time males and females are equally affected 2528. Patients with HF average at least two hospital admissions per year 25,29,30. Among patients who are admitted with AHF, over 80% have a prior history of HF, referred to as decompensated heart failure 2023.…”
