Neuropathic arthropathy, also known as Charcot arthropathy, refers to progressive and occasionally rapid joint destruction that results from underlying disorders of the nervous system. This pictorial essay aims to illustrate various radiologic findings in neuropathic arthropathy using various examples in the upper and lower limbs and in the spine. Pearls for radiologic diagnosis, clinical differential considerations and possible complications are discussed individually for each joint. MR imaging techniques for differentiating infection and neuropathic arthropathy are explained with examples. Management issues are outlined.

Türk Ortopedi ve Travmatoloji Birliği Derneği sıklıklarının bilinmesi; hasta seçimi, kullanılacak cerrahi teknik, seçilecek implant, hasta güvenliği ve fonksiyonel sonuçlar açısından önem arz etmektedir. Bu makalede, primer osteoartit dışı nedenlerle yapılan primer total diz protezlerinde görülebilecek komplikasyonlar ve literatürdeki olgu serileri tartışılacaktır. ROMATOİD ARTRİT HASTALARINDA GÖRÜLEN KOMPLİKASYONLAR Dizin inflamatuvar artrit patogenezi, medikal tedavi, prognoz ve beklenen sonuçları açısından osteoartritten farklılık gösterir. Romatoid artrit inflamatuvar artritler arasında en sık görülendir. Romatoid artrit, otoimmün sinovit zemininde kıkırdak harabiyeti ile sonuçlanan bir hastalıktır. İleri evre romatoid artrit hastalarında diz eklemi %90'a varan oranlarla etkilenir. [2] Primer TDA uygulanan hastaların %8'lik bir bölümünü K omplikasyon, bir hastalığın seyrinde görülen morbid bir süreç veya olay şeklinde tanımlanır. Cerrahi komplikasyonlar; cerrahi sırasında veya sonrasında gelişenler olarak gruplandırılabilir. Cerrahi komplikasyonların bilinmesi ve farkında olunmasıyla, bunlarla karşılaşma oranları azaltılabilir. [1] Primer total diz artroplastisi (TDA) son yüzyılda gerçekleşen en başarılı cerrahi işlemlerden birisidir. TDA günümüzde sıklıkla primer osteoartrit hastalarına uygulanmaktadır. Primer osteoartrit dışında ise romatoid artrit, hemofilik artropati, yüksek tibial osteotomi sonrası gelişen artrit ve nöropatik artropati nedeniyle primer TDA uygulanabilmektedir. Literatürde primer osteoartrit daha sık görüldüğünden, komplikasyonları inceleyen çalışmalar da sıklıkla primer osteoartrit üzerinedir. Zusammenfassung Hintergrund Während die Charcot Neuroarthropathie (CN) des Fußes immer mehr Aufmerksamkeit gewinnt, wird die CN des Knies kaum beachtet. Zwar erschienen in den letzten Jahren einige Publikationen zu CN des Knies, es handelte sich aber fast ausschließlich um Einzelfallbeschreibungen und meist dann auch nur zur endoprothetischen Versorgung. Es existieren nur 2 systematische Beschreibungen der Erkrankung. Ergebnisse Es wird an Hand eines Patienten nun der Krankheitsverlauf und die Charakteristika der CN am Knie detailiert sowie die Therapieoptionen dargestellt und in den Kontext einer vorangegangenen Publikation mit 3 Patienten mit CN des Kniegelenkes gesetzt. Die CN des Kniegelenkes zeigte die gleiche schubartige und stadienhafte Klinik analog zur CN des Fußes. Die Anwendung derselben Therapieprinzipien wie bei der CN des Fußes (sofortige Immobilisierung und Offloading) führte in allen Fällen zu einer schnellen Überführung in ein inaktives Stadium mit Konsolidierung des Befundes, sodass alle Patienten mittels Orthesen und ohne Operation wieder gehfähig wurden. Es handelte sich bei allen Patienten um Diabetiker mit multiplen Komplikationen (3x Diabetes Typ I, 1x Diabetes Typ 2), alle litten an einer stark fortgeschrittenen Polyneuropathie der unteren Extremität. Schlußfolgerung Die CN des Kniegelenkes stellt eine schwere Komplikation bei Polyneuropathie dar und darf nicht übersehen werden. Dieselben Diagnose- und Behandlungskriterien wie bei der CN des Fußes führten zum Erhalt von Knie und Gehfähigkeit.
