Background: Despite the well-documented role of cigarette smoke in the development of chronic obstructive pulmonary disease (COPD), lung cancer and cardiovascular disease, biomarkers for screening or monitoring disease progression and outcome remain elusive, particularly for COPD and lung cancer. Inflammatory cells and mediators are likely to be involved in the disease processes, but their importance is still poorly understood. The purpose of this study was to investigate early changes in immunological markers associated with smoking in healthy monozygotic twins without a detectable disease discordant for smoking, thereby minimising data variability due to genetic background. Methods: Twenty-two monozygotic twin pairs, aged 31.5 ± 6.3 years, entered the study. One of each twin pair was a smoker and the other a non-smoker. None of the subjects reported any diseases or clinically defined respiratory symptoms or airflow limitation. Each subject donated blood samples for determination of total leukocytes and subpopulations, lymphocyte subpopulation plus pro-inflammatory mediators (interleukin-8, tumour necrosis factor-α, soluble tumour necrosis factor-α receptors and C-reactive protein). Results: We observed a significant increase in the number of circulating leukocytes and neutrophils in smokers compared to non-smokers. Smokers also had significantly higher numbers of B cells and CD4 + T cells, plus an increased CD4/CD8 ratio. The numbers of NK cells were statistically significant lower in smokers compared to non-smokers.

Abstract:The relationship between biomarkers of exposure to cigarette smoke in 24h urine samples collected from groups of 80 smokers (44 males, 36 females) and 40 never smokers (17 males, 23 females) at two centers in Europe was studied. Eight biomarkers (nicotine, cotinine, hydroxycotinine, 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanol (NNAL) and all of the respective glucuronide conjugates) were measured. Subjects from the two centers were pooled and biomarker data analyzed according to the machine smoked tar yield of the brand each subject smoked and the recorded number of cigarettes smoked per day (CPD). A statistically significant relationship between CPD and all of the biomarkers analyzed was found. Smokers of less than 11 CPD had the lowest mean 24h urinary concentrations for all biomarkers measured. However, if the amount of constituent obtained from each cigarette smoked was calculated, then the amount of nicotine obtained per cigarette was highest in this group although the variation was also greatest for this group. The amount of NNK (4-(methylnitrosamino)-1-(3-pyridyl)-1 butanone, the parent molecule of NNAL) obtained per cigarette was not statistically significantly different across all groups. In conclusion, these results confirm the reliability of 24h urinary total nicotine and NNAL concentrations as biomarkers of exposure to specific cigarette smoke constituents across two centers in Europe. These measurements may provide an objective alternative to CPD when grouping smokers for are studies of other endpoints.

Swift growth in the use of LC-MS/MS for the analysis of drugs in biological matrices has been compelled by the need for timely and high-quality data at many stages in drug discovery and development process: from high throughput screening of drug candidates and rapid data generation for pre-clinical studies to almost 'real-time' analysis of clinical samples. Prompt and rational method development, validation, and transfer play a pivotal role in achieving the goals of "faster, better, and cheaper" for pharmacokinetic studies since this could easily account for more than 50% of the time and labor resources for a moderate-sized project. Strategy for rational method development, validation and transfer has been largely kept as institutional knowledge but rarely appeared in literature. In this review article, strategies for developing and validating robust high throughput LC-MS/MS methods will be critically reviewed and discussed. Automated sample preparation, fast chromatography, minimization of matrix effects, and strategy of narrowing the gap between validation and incurred sample analysis are just a few topics covered in this review. Other interesting approaches for improving method efficiency and ruggedness such as direct injection SPE and liquid/liquid extracts as well as multiplexing of LC columns will also be discussed. Potential pitfalls during method development and validation are pointed out. At the end, the question "how fast is fast enough and how fast is too fast?" will be answered after considering all aspects of the method development and validation.
