Objective To investigate the short-and long-term efficacy and safety of infliximab (IFX) in intestinal Behçet's disease (BD) patients in a retrospective cohort study. Methods Among 43 consecutive patients with intestinal BD presenting at the same clinic, 15 with active disease and receiving standard treatment were given IFX infusions (5 mg/kg body weight) every eight weeks. The patients were clinically and endoscopically evaluated before treatment, then assessed after 10 weeks, 12 months and 24 months for a clinical response, defined as a significant improvement in intestinal symptoms and a reduced C-reactive protein (CRP) level. Results At week 10, 12 patients (80%) exhibited a response to IFX, with eight (53%) in remission with no intestinal symptoms and normal CRP levels. A response to IFX was maintained in seven of the 11 patients (64%) available at 12 months and in four of the eight patients (50%) available at 24 months. Of the seven patients receiving prednisolone at entry, five responders had their steroid doses reduced. Fulminant intestinal BD was predictive of an absence of response to IFX. The adverse effects comprised one infusion reaction and one case of fever, most likely related to IFX. Conclusion IFX is effective and safe in patients with refractory intestinal BD.

A két szerző a közlemény létrehozásában egyenlő arányban vett részt.A Behcet-kór multiszisztémás autoimmun betegség változatos klinikai megjelenési tünetekkel. Hazánkban a Behcetkór a ritka betegségek közé tartozik, a témával foglalkozó közlemények száma elenyésző. Történelmünk során a népcsoportok keveredése, valamint a világszerte növekvő prevalencia és a szerzők tapasztalata is arra utal, hogy a betegség előfordulására az eddig vártnál nagyobb gyakorisággal kell számítani. A szerzők felhívják a fi gyelmet erre a sokoldalúan megjelenő, sok szakterületet érintő betegségre. A diagnózis nagy kihívást jelent a klinikus számára, akinek tisztában kell lennie a lehetséges tünetek spektrumával és kombinációival. Irodalmi adatok és saját tapasztalataik alapján a szerzők össze foglalják a betegség eddig ismert patogenetikai és patomechanikai tényezőit és a diagnó-zishoz nélkülözhetetlen szimptómák rendszerét, a kezelés lehetőségeit. A közleménnyel a szerzők a diagnosztikát, a magyar betegpopuláció feltérképezését és jobb kezelését szeretnék segíteni, és kezdeményezik hazai Behcetregiszter felállítását. Orv. The diagnosis may pose a diffi cult challenge for the clinician, who has to be familiar with the wide spectrum and combination of the symptoms of Behcet's disease. It is considered a rare disease in Hungary, and there are only few reports on Behcet's disease in the Hungarian literature. However, the past history of Hungary, the worldwide growing incidence of the disease, and the authors' experience raise the possibility that the occurrence of the disease is higher than previously thought. In this review the authors present and discuss literature data on the pathogenesis and pathomechanism, as well as their own experience concerning the symptomatology of Behcet's disease in order to promote diagnosis and offer adequate therapy for the patients. The authors presume that the importance of the disease is underestimated in Hungary due to a considerable number of unrecognized cases and they propose to establish a national registry for Behcets disease. Behçet's disease is a complex disease characterized by remission and activation periods of unknown duration. It has an unpredictable course. Behçet's disease shows a heterogeneous patern of organ involvement that occurs in recurrent episodes of acute inlammation throughout the course of the disease. Disease activity in Behçet's disease is diicult to deine because of its luctuating course, lack of laboratory tests relecting overall disease activity, absence of a standardized form to report the severity of Behcet's disease manifestations and also trying to develop new diagnostic criteria. This led to the development of standardized disease activity index. To be useful, a measurement of disease activity must be valid, reliable, and simple enough to use in routine clinical practice. We will try to explain what the situation is in terms of Behçet's disease activity index.
