Objective: Laparoscopic surgery has become increasingly popular in urology over the last decade. This survey was performed to evaluate the current practice patterns in laparoscopy among urologists in Turkey. Material and methods:A detailed questionnaire about urologic laparoscopic practice patterns was distributed to 1242 urologists who were working in Turkey. The questions pertained to age, practice demographics, and the amount and variety of laparoscopy performed.Results: Nearly half of the respondents (48.3%) already performed laparoscopy. Of the urologists in academic settings, including university hospitals and education and research hospitals, 69.6% and 59.4% performed laparoscopy, respectively. In state hospitals, the percentage of those who performed laparoscopy was lower (26.9%). The most important reasons mentioned for performing laparoscopy were shorter hospital stay, patient requests and greater flexibility of possible surgical techniques. The main laparoscopic procedures performed were nephrectomy (benign indication), 91%; renal cyst decortications, 90%; nephrectomy (malign indication), 65%; laparoscopic stone surgery, 47% and pyeloplasty, 38%. A large percentage (77.2%) of respondents intended to attend continuing education and to perform laparoscopy in the future. Conclusions:Our results show that laparoscopic practice steadily increases in frequency and that urologists are willing to substitute open surgery for laparoscopic surgery. The performance of laparoscopic surgery has become a goal for most urologists in Turkey to achieve in the near future.Key words: Laparoscopy; questionnaire; survey; urology. ÖZETAmaç: Laparoskopik cerrahi üroloji alanında son on yılda giderek popüler olmuştur. Bu anket Türkiye'deki ürologların laparoskopi alanındaki mevcut pratiklerini araştırmak amacıyla gerçekleştirildi. Gereç ve yöntemler:Ürolojik laparoskopi konusunda hazırlanan ayrıntılı bir anket formu Türkiye'de ça-lışan 1242 ürologa e-mail yoluyla gönderildi. Sorular, ürologun yaşını, çalıştığı yerin özelliklerini, yaptığı laparoskopik ameliyatların çeşitlilik ve miktarını kapsamaktaydı. Bulgular: Ankete cevap verenlerin yaklaşık yarısı (%48.3) zaten laparoskopi uygulamakta olduğunu belittiler. Üniversite hastanesi ve eğitim-araştırma hastanesi gibi akademik ortamlarda bulunan ürologların sırasıyla %69.6 ve %59.4'ü laparoskopi uygulamaktaydı. Devlet hastanesinde ise laparoskopi uygulayanların oranı daha düşüktü (%26.9). Laparoskopi uygulamak için öne sürülen en önemli nedenler; kısa hastanede kalış süresi, hastaların tercihi ve daha fazla cerrahi teknik sunabilmedir. Évaluation clinique de l'efficacité de l'entraînement sur simulateur à la réalisation de noeuds intracorporels par les internes de chirurgie : un programme mis en place dans un service de chirurgie pédiatrique Clinical evaluation of intracorporeal knot tying simulations by surgical residents: a pediatric surgery study Isabelle Abstract -Objective: Learning endoscopic procedures in the operating area during surgery residencies is becoming increasingly challenging even though more and more laparoscopic procedures can be carried out. As a result, additional complications may occur due to a lack of training. We evaluated the clinical impact of a laparoscopic training program on acquiring basic laparoscopic skills, such as intracorporeal knot tying in pediatric surgery centres. Material and methods: Fourteen surgery residents were assigned in two randomised groups: a training group (n = 7) and a control group (n = 7). Each resident carried out an initial endoscopic intracorporeal knot during a laparoscopic inguinal hernia repair in a child followed by a second knot several weeks later. The control group did not perform any other endoscopic intracorporeal knot tying during the study. The training group benefited from pelvic area training. Each knot was recorded. In addition, the time it took to make each knot and the quality of each procedure were analysed. Results: The criteria were improved for the second knot in the training group while it was not in the control group. Conclusion: Laparoscopic training on simulators helps speed up the basic skill learning curve of surgical residents and has a real impact on clinical practice. Moreover, patients benefit directly from this type of training since there are few complication risks in the early part of learning.

Background:The expanding use of advanced minimally invasive surgical techniques demands more advanced training methods, objective measures of resident performance, and more realistic and anatomically correct training models.Materials and Methods: A new synthetic torso for urologic laparoscopy training was developed and assessed. The trainer, Lapman, was based on the Visible Human Model and has the exact shape of a human torso. The torso models the outer shape of the body and the abdominal and pulmonary cavities. Animal or synthetic models of the abdominal organs may be placed in the abdominal cavity. An abdominal wall provides access and seals the cavity and can be replaced after repeated punctures with laparoscopic instruments. The thoracic cavity connects to a pneumatic pump to simulate breathing. In order to render realistic mechanic properties, the torso is cast of materials with elastic properties similar to those of soft tissue and incorporates a synthetic skeleton. These similar mechanical properties and the thoracic insufflation create realistic ventilatory motion simulation.Results: Twenty-five individuals-medical students, residents, and attending urologists-participated in a study comparing Lapman with a standard training box. Lapman presented several advantages over the traditional training box, specifically with regard to internal and external views and the incorporation of a realistically shaped abdominal wall. A significant and recurrent theme was the value of the synthetic wall as a tool to gain a greater appreciation of the importance of port placement. Study participants at all levels of training appreciated that Lapman gives a more realistic approximation of the operative procedure.Conclusions: The novelty of the trainer consists in its anatomic shape, realistic mechanical properties, and ventilatory simulation. This paper reports on its design, construction, and preliminary tests.
