Development of a novel multichannel port (x-Gate®) for reduced port surgery and its initial clinical results

Abstract: Overall performance of x-Gate® was sufficient in the clinical experience. There have been no complications attributed to x-Gate®. We found that with the x-Gate® the conflicts among the forceps have been drastically improved compared with other multi-channel ports we had used before, which had a shorter distance between the channels.

“…Consequently, an inherent technical challenge that arises from the SILS technique is that of a compromised view and locomotive field [ 21 ]. A small additional port or reduced number of ports (reduced port surgery, RPS) are modifications that may overcome these problems [ 7 , 22 ]. Our dual-port method provides an additional 12-mm port for the surgeon’s right hand.…”

“…Multi-channel ports, which enable insertion of multiple instruments via a single incision, are now available worldwide. Recently, single-incision laparoscopic surgery (SILS) or reduced port surgery (RPS), using a multi-channel port to achieve less invasive surgery has been reported [ 7 , 8 ]. A number of instruments are available to support this procedure.…”

“…From the view point of development of its approach route, when PEIGS was invented in 1993 three ports were inserted into the stomach, while we developed single incision intragastric surgery with utilization of a multichannel port (9,10) in 2011 for better cosmetic effect and better facilitation of specimen extraction (11). We have reported satisfactory short-and longterm clinical outcomes of PEIGS to justify this operation from the oncological point of view (12).…”

“…Reduced port surgery has been developed to further minimize the invasiveness of laparoscopic surgery (1)(2)(3). This challenge was attempted also for laparoscopic transabdominal preperitoneal herniorrhaphy (TAPP), and thus both single incision TAPP (4,5) and needlescopic TAPP (2,6) have been developed .…”
