PurposeTo compare the oncologic outcomes of T2 bladder cancer (BCa) patients after partial cystectomy (PC) or radical cystectomy (RC).Materials and MethodsRelevant data of T2 BCa patients diagnosed between 2004 and 2015 were retrieved from the Surveillance, Epidemiology, and End Results database. After propensity score matching, the oncologic outcomes after PC or RC were compared in patients who underwent lymph node dissection (LND) or not. Additionally, the benefits of different LND strategies were also explored.ResultsEventually, 3834 T2 BCa cases were enrolled in the analysis, including 724 (18.9%) received PC cases and 3110 (81.1%) RC cases. The PC and RC groups presented entirely different profiles in clinical parameters such as age, number of lymph nodes (LNs) removed, and adjuvant therapy. Particularly, LNDs were performed in 92.0% of RC cases, while only in 45.4% of PC cases. After propensity score matching, PC and RC present similar oncologic outcomes regardless of the LND strategies. Further exploration found that LND could improve the prognosis of patients and the benefit is associated with the number of LNs removed.ConclusionPC and RC could provide equivalent oncologic outcomes in T2 BCa. LND is essential in curative surgery and could significantly affect the prognosis, but it was frequently neglected, especially in PC. The criteria for PC in T2 BCa need further exploration in future studies.

Background Ovarian mucinous carcinoma is a disease that requires unique treatment. But for a long time, guidelines for ovarian serous carcinoma have been used for the treatment of ovarian mucinous carcinoma. This study aimed to construct and validate nomograms for predicting the overall survival (OS) and cancer-specific survival (CSS) in patients with ovarian mucinous adenocarcinoma. Methods In this study, patients initially diagnosed with ovarian mucinous adenocarcinoma from 2004 to 2015 were screened from the Surveillance, Epidemiology, and End Results (SEER) database, and divided into the training group and the validation group at a ratio of 7:3. Independent risk factors for OS and CSS were determined by multivariate Cox regression analysis, and nomograms were constructed and validated. Results In this study, 1309 patients with ovarian mucinous adenocarcinoma were finally screened and randomly divided into 917 cases in the training group and 392 cases in the validation group according to a 7:3 ratio. Multivariate Cox regression analysis showed that the independent risk factors of OS were age, race, T_stage, N_stage, M_stage, grade, CA125, and chemotherapy. Independent risk factors of CSS were age, race, marital, T_stage, N_stage, M_stage, grade, CA125, and chemotherapy. According to the above results, the nomograms of OS and CSS in ovarian mucinous adenocarcinoma were constructed. In the training group, the C-index of the OS nomogram was 0.845 (95% CI: 0.821–0.869) and the C-index of the CSS nomogram was 0.862 (95%CI: 0.838–0.886). In the validation group, the C-index of the OS nomogram was 0.843 (95% CI: 0.810–0.876) and the C-index of the CSS nomogram was 0.841 (95%CI: 0.806–0.876). The calibration curve showed the consistency between the predicted results and the actual results, indicating the high accuracy of the nomogram. Conclusion The nomogram provides 3-year and 5-year OS and CSS predictions for patients with ovarian mucinous adenocarcinoma, which helps clinicians predict the prognosis of patients and formulate appropriate treatment plans.

Background. Noncancer death accounts for a high proportion of all patients with bladder cancer, while these patients are often excluded from the survival analysis, which increases the selection bias of the study subjects in the prediction model. Methods. Clinicopathological information of bladder cancer patients was retrieved from the Surveillance, Epidemiology, and End Results (SEER) database, and the patients were categorized at random into the training and validation cohorts. The random forest method was used to calculate the importance of clinical variables in the training cohort. Multivariate and univariate analyses were undertaken to assess the risk indicators, and the prediction nomogram based on the competitive risk model was constructed. The model’s performance was evaluated utilizing the calibration curve, consistency index (C index), and the area under the receiver operator characteristic curve (AUC). Results. In total, we enrolled 39285 bladder cancer patients in the study (27500 patients were allotted to the training cohort, whereas 11785 were allotted to the validation cohort). A competitive risk model was constructed to predict bladder cancer-specific mortality. The overall C index of patients in the training cohort was 0.876, and the AUC values were 0.891, 0.871, and 0.853, correspondingly, for 1-, 3-, and 5-year cancer-specific mortality. On the other hand, the overall C index of patients in the validation cohort was 0.877, and the AUC values were 0.894, 0.870, and 0.847 for 1-, 3-, and 5-year correspondingly, suggesting a remarkable predictive performance of the model. Conclusions. The competitive risk model proved to be of great accuracy and reliability and could help clinical decision-makers improve their management and approaches for managing bladder cancer patients.
