Development and Validation of a Semiquantitative Brain Maturation Score on Fetal MR Images: Initial Results

Abstract: Purpose:To develop a valid, reliable, and simple-to-use semiquantitative visual scale of fetal brain maturation for use in clinical fetal MR imaging assessment and interpretation. Materials and Methods:This is a retrospective assessment of data from a previous study that was prospective, institutional review board approved, and HIPAA compliant. Forty-eight normal pregnancies with a gestational age (GA) of 25 to 35 weeks were studied. A fetal total maturation score (fTMS) was developed by utilizing six subscore… Show more

“…A single fetal total maturation score (fTMS) was calculated by summing up 6 semiquantitative subscores characterizing various visual sulcation observations, the extent and location of myelination, and evolution of the germinal matrix. 16 We found that the folding measures obtained by the proposed framework were in high agreement with the average visual fTMS (Fig 2). …”

“…A cohort of 33 healthy fetuses with high-resolution reconstructed MRI was included in this study, of which 10 subjects were female and 23 were male. The cohort has been previously reported in studies of normative brain development 16 but has not been used to estimate gestational age.…”

“…Ventricular size was measured at the atrial level. The fetal total maturation score (fTMS), which is a validated semiquantitative fetal brain maturation score using various cortical sulcation, myelination, and germinal matrix subscores [1] , was used to provide a measurement of brain maturity on the fetal MR images.…”

“…We prospectively enrolled healthy volunteers with singleton pregnancies and a normal pregnancy history, with normal clinical laboratory and ultrasound studies during the current pregnancy [12,25,[27][28][29] . Exclusion criteria included multiple gestations, known or suspected genetic or chromosomal abnormalities, as well as any maternal contraindication for MRI, including physical (e.g., metal implants) or psychosocial (e.g., claustrophobia) contraindications.…”
