Development and Validation of a Scoring System for Differential Diagnosis of Tuberculosis and Metastatic Tumor in the Spine

Abstract: Purpose Spinal tuberculosis (TB) and metastatic tumor (MT) are common diseases with similar manifestations. Although pathological evaluation is the gold standard to confirm diagnosis, performing biopsies in all patients is not feasible. This study is aimed to create a scoring system to facilitate the differential diagnosis of spinal TB and MT before invasive procedures. Methods Altogether, 447 patients with spinal TB (n=198) and MT (n=249) were retrospectively analyzed.… Show more

“…Approximately 10% of metastases occur in the spine [ 27 ]. MSCC is a severe oncological emergency that often leads to neurological sequelae and even permanent disability [ 1 , 28 ]. Once MSCC occurs in patients with spinal metastases, their quality of life is extremely affected.…”

“…10 With regard to laboratory parameters, given that ESR and CRP levels are elevated in most tuberculosis patients, they can be used as an initial identification method. 85 On Xray, osteolytic or osteogenic changes in metastatic lesions can be observed only if more than half of the bone areas is destroyed, 84 but pedicle cortex destruction is often an early sign of spinal metastases. For MRI imaging, most metastatic lesions are seen to be hypointense on T1WI, hyperintense on T2WI and STIR, and show heterogeneous enhancement with gadolinium.…”
