Purpose Musicians' practice and performance routines reportedly lead to musculoskeletal complaints and disorders (MCD) that impact their wellbeing and performance abilities. This systematic review aims to assess the prevalence, risk factors, prevention and effectiveness of treatments for MCD in professional musicians and consider the methodological quality of the included studies. Methods A systematic literature search was performed in December 2017 using electronic databases and supplemented by a hand search. Case-control studies, cohort studies, cross-sectional studies, interventional studies and case reports investigating the prevalence, risk factors, prevention or treatment effects of MCD in professional musicians or music students (age ≥ 16 years) were included. Quality assessments of the included studies were performed using an adapted version of the "Study Quality Assessment Tools" from the National Heart, Lung, and Blood Institute. Results One case-control study, 6 cohort studies, 62 cross-sectional studies, 12 interventional studies and 28 case reports were included and assessed for methodological quality. The study designs, terminology, and outcomes were heterogeneous, as the analyses mostly did not control for major confounders, and the definition of exposure was often vague. Therefore, evidence that being a professional musician is a risk factor for MCD as well as the causal relationship between these factors remains low despite the fact that a large number of studies have been performed. Conclusions Studies with high internal and external validity regarding the prevalence, risk factors and effectiveness of the prevention or treatment of MCD in professional musicians are still missing. Further high-quality observational and interventional studies are required.

The authors aimed to characterize the current evidence base regarding musicians' musculoskeletal symptoms (MSS), in order to identify gaps for future research. A systematic search was conducted to identify peer-review studies published in English in 2007-2016 that investigated musicians' MSS. Narrative reviews, case reports, protocols, and questionnaire or program development papers were excluded. Data were synthesized descriptively in order to identify gaps in the current evidence base. Five systematic reviews and 153 primary studies (133 unique) were included in this review. The majority (71%) of studies investigated professional musicians and/or university music students, with orchestral musicians being the most commonly investigated group. The majority of studies investigated the extent of the problem (68%) and/or associated factors (54%). Eight studies compared the prevalence of MSS outcomes with other populations. A range of risk factors were investigated; however, few studies used longitudinal designs. A total of 16 intervention studies were identified (3 clinical, 13 public health), with 12 investigating education or exercise programs. There is a need for research into musicians beyond classical university music students and professional orchestral musicians, and these musical subgroups should be compared to determine the most at risk groups of musicians. Studies looking at potential risk factors should move towards longitudinal designs so that the temporal relationship of these factors and MSS could be established, where cross-sectional designs have indicated that an association exists. Intervention studies should be based upon the risk factors identified, and extend beyond education and exercise programs.

Trata-se de um relato de experiência cujo objetivo foi apresentar o Programa de Atenção Integral a Saúde do Artista de Performance do Serviço Especializado em Saúde do Trabalhador do Hospital das Clínicas da Universidade Federal de Minas Gerais e relatar as ações desenvolvidas pela equipe interdisciplinar. As ações compreendem avaliação clínico-ocupacional, incluindo análise da atividade com o instrumento musical; visitas técnicas aoslocais de trabalho, assistência individual ou atividades em grupo e, ações educativas e preventivas, como a elaboração de um folheto informativo e workshops sobre a saúde do músico. De dezembro de 2009 a maio de 2015 foram atendidos 122 músicos, sendo amaioria instrumentistas, do sexo masculino, com média de idade de 32 anos. As reavaliações e os relatos dos trabalhadores assistidos demonstraram que os objetivos do programa vêm sendo alcançados, principalmente no que diz respeito ao autogerenciamento das estratégias de enfrentamento dos riscos de adoecimento no trabalho. Para dar sustentação científica aos resultados, foi elaborado um protocolo sistematizando as atividades em grupo, que se encontra em fase de avaliação. Espera-se que a experiência do SEST com relação a saúde do músico possa auxiliar outros profissionais e serviços a se organizar frente as demandas deste coletivo.
