Development and Management of Severe Cutaneous Side Effects in Head-and-Neck Cancer Patients during Concurrent Radiotherapy and Cetuximab

Abstract: These findings indicate that cetuximab may have the potential to enhance the severity of radiation dermatitis in HNC patients. A systematic monitoring of cutaneous side effects during radiotherapy plus cetuximab is advised in order to reliably estimate the frequency of severe (grade 3/4) radiation dermatitis.

“…Notably, investigations have shown that about 20% of acute renal failure cases among hospitalized patients are due to platinum-based chemotherapy [4,19,22,27]. Several authors agree that renal damage due to cisplatin is irreversible.…”

“…Despite the fact that EGFRI-induced cutaneous adverse effects were initially reported to be sterile [7], there is now accumulating evidence that the use of EGFRI is associated with the development of infectious complications [13,14,15,16,17,18,19,20]. Strikingly, a recent study by Lacouture [3] reported that bacterial infections were found at sites of skin toxicities in approximately one third of cancer patients treated with EGFRI.…”

“…Late toxicity without total loss of function but with severe impact on the patient's quality of life, such as severe limitation of oral intake caused by fibrosis, strictures, or stiffness of the temporomandibular joint, was also considered as severe radiation-induced late toxicity. In order to describe the cetuximab-related acneiform rash, the following clinical grading system (NCI-CTC) was used: no (grade 0), light (grade 1), moderate (grade 2) and severe (grade 3) acnelike skin reaction [4,25].…”
