Background: Colour doppler is non-invasive, cheaper and faster modality for evaluation of extra cranial course of carotid vessels. In evaluation of the extra cranial territory and its accuracy colour Doppler sonography was well established in comparison with angiography. Aim was to study the extra cranial carotid vessels by using colour doppler sonography.Methods: A total of 80 patients were examined by duplex sonography who were attending the medical department of Osmania medical college and hospital, Hyderabad, Telangana, India, from June 2014 to December 2015.Results: Highest incidence of the carotid disease is in the age group of 30 to 60 years. Out of 21 positive patients 13 patients were males and 9 were females. Male to female ratio is 1.5:1. Spectral trace was obtained in all the patients and cardinal four velocity parameters were measured. Takayasu’s arteritis is found to be more common carotid disease second to atherosclerosis in present study.Conclusions: Principal cause of carotid disease in Present study is atherosclerotic plaques. Atherosclerotic plaques are mostly found at the level of carotid bifurcation and the internal carotid artery. We suggest that colour doppler is significant advancement in carotid diagnosis.

Purpose: To evaluate the role of computed tomography angiography (CTA) and magnetic resonance angiography (MRA) in the quantification of atherosclerotic stenosis of carotid artery bifurcation in comparison with digital substraction angiography (DSA) and Doppler sonography (DS). Materials and Methods: Twenty-five patients with atherosclerotic disease of the carotid arteries with proven stenosis by DSA, had spiral CTA, MRA using two- and three-dimensional time-of-flight gradient echo techniques, and DS using Doppler flow signal recording (total 47 carotid artery bifurcations). The degree of stenosis was measured according to the North American Symptomatic Carotid Endarterectomy Trial criteria: total occlusion (100%), severe (70–99%), moderate (30–69%) and mild (0–29%). The degree of stenosis measured by CTA, MRA and DS was compared to DSA, used as the gold standard. Results: Ninety-seven percent of MRA measures were equivalent to DSA, and 3% were underestimated; 96% of CTA measures were equivalent to DSA, and 4% were underestimated; 77% of DS measures were equivalent to DSA, 21% were overestimated and 2% were underestimated. Conclusions: CTA and MRA are equally accurate methods in quantifying the degree of carotid bifurcation stenosis.

Background and Purpose-We hypothesized that direct cervical investigation with Power M-Mode Doppler (PMD) combined with single-gate Doppler spectral analysis (SGDSA) using a 2-MHz pulsed-wave Doppler transducer would show reasonable accuracy parameters when compared with standard color-coded carotid duplex ultrasound (CDU). Methods-We prospectively screened for cervical internal carotid artery (ICA) stenosis by direct observation using a 2 MHz PMD/SGDSA device. PMD identified the artery (location, depth, flow direction) and SGDSA assessed waveform; peak systolic, end diastolic, and mean flow velocities (MFV) of the common carotid artery; cervical ICA proximally and distally; and external carotid artery. Diagnostic accuracy was compared with concurrent carotid duplex ultrasound. The continuity principle was applied using the proximal/distal cervical ICA MFV ratio. Results-We examined 456 vessels (228 patients). Using ICA proximally/ICA distally MFV ratio of 1.5 or greater or absence of ICA signature, for 40% to 59% or greater stenosis (including occlusions), sensitivity was 75.4%, specificity 99.8%, positive predictive value 97.7%, negative predictive value 96.6%, and accuracy 96.7%. For MFV ratio 1.6 or greater or absence of ICA signature and 60% to 79% or greater stenosis (including occlusions), sensitivity was 92.3%, specificity 98.1%, positive predictive value 81.8%, negative predictive value 99.3%, and accuracy 97.6%. Conclusions-Use of combined PMD and SGDSA to directly observe the extracranial ICA is reasonably accurate compared with carotid duplex ultrasound. Using the MFV ratio of proximal/distal extracranial ICA improves accuracy parameters and provides a quick and effective bedside screen for ICA stenosis. This novel technique should be considered part of the standard PMD/transcranial Doppler examination.
