Determination of sixty percent or greater carotid artery stenosis by duplex Doppler ultrasonography

Abstract: It is concluded that 60% or greater carotid artery stenosis can be reliably determined by duplex criteria. The use of receiver-operator characteristic curves allows the individualization of duplex criteria appropriate to specific clinical situations of patient screening for lesions (high sensitivity and NPV) or use as a sole preoperative imaging modality (high PPV). Individual vascular laboratories must validate their own results.

“…Specificity of such examination is 75% and positive predictive value is of 72%. 8 In present study we have noted the PSV ratio is less than 1.5 for stenosis less than 40% in eight patients. Three patients with 70 to79% stenosis has PSV ratio greater than 1.8.…”

“…The NASCET published criteria for velocity values predicting 70–99% stenosis show PSV values ranging between 130 and 325 cm/s, the best parameter being an end diastolic velocity >100 cm/s and an internal carotid artery PSV/common carotid artery PSV ratio >4.0 [33, 34]. The best ACAS-published criteria for velocity values predicting 60–99% stenosis is a PSV value >170 cm/s [35]. …”

“…Separate local Intersocietal Commission for the Accreditation of Vascular Laboratories (ICAVL)-certified ultrasound laboratories performed the CDU studies using a standard insonation protocol and diagnostic criteria 3,4 by means of an ATL 5000 ultrasound system with a linear 7-to 4-MHz probe. The results were expressed as a percent stenosis range: 0%, 1% to 39%, 40% to 59%, 60% to 79%, 80% to 99% stenosis, and occlusion.…”

“…Reliable noninvasive tests are needed to detect internal carotid artery (ICA) disease in high-risk patients. [3][4][5] Rapid diagnosis of significant carotid stenosis and occlusion is important for the management of patients, especially in the setting of acute stroke. Transcranial Doppler (TCD) is noninvasive, inexpensive, convenient, portable, and can quickly provide a global image of the hemodynamic status of cerebral circulation in patients with stroke as a complementary test to refine other imaging findings and detect tandem lesions.…”
