1 Ten patients with bilateral knee joint effusions were treated topically with a gel containing 1 g diclofenac/100 g (80 mg three times daily). They were randomized to receive diclofenac gel to one knee and a placebo gel preparation to the other knee. 2 Diclofenac was assayed in synovial fluid and blood plasma by GC/ECD as the pentafluorobenzyl-ester derivative. 3 Total concentrations of diclofenac in synovial fluid (day 4) were significantly higher in the diclofenac gel treated knee than in the contralateral placebo treated knee (25.5 ± 3.6 ng ml-1 vs 21.6 ± 2 ng ml-1; P < 0.05). These concentrations were lower than total plasma drug concentrations (40.6 ± 4.7 ng ml-', n = 10, P < 0.01). Unbound concentrations of diclofenac in synovial fluid from either the diclofenac gel treated or the placebo treated knee were not significantly different from each other or from plasma free concentrations (115 ± 16 and 99 ± 12 vs 108 ± 19 pg ml-'). 4 Clinical parameters showed improvement of joint mobility and a small reduction of swelling (circumference) in both knees with time. However, the differences between knees were not significant. 5 We conclude that direct transport of diclofenac from the skin into the ipsilateral knee joint after cutaneous application is minimal. Distribution seems to be predominantly via the blood. Whether the observed improvements of clinical parameters were due to drug effects or to the spontaneous course of the underlying disease cannot be distinguished.

This paper reports, for the first time, the data on the pharmacokinetic characteristics of diclofenac sodium and cefadroxil at Albaha, 2500 M altitude (ALT) , KSA. Therefore, the objective of this study was to compare the plasma concentrations and pharmacokinetic parameters of diclofenac sodium (Voltaren  50 tablets, Ciba Geigy, USA) and cefadroxil (Ultrcef  500 capsules, Bristol L. Germany) at altitude of 2500 M (ALT) and at sea level (SEA) after oral administration to 12 healthy volunteers. A two-way cross over study design was used to compare pharmacokinetic parameters at ALT and SEA. Following drug administration, blood withdrawn into heparinized test tubes over a period of 12 hours. Drug concentrations were determined in the withdrawn samples by fully validated and optimized HPLC methods for the two drugs. The pharmacokinetic parameters including C max , t max , t 0.5e , t 0.5a , K e , K a , V d , Cl T , AUC 0-12h , and AUC 0-∞ were determined by a computer programs. Statistical analysis of the obtained plasma drug concentrations and pharmacokinetic parameters were performed using ANOVA computerized system. Results obtained showed significant differences in the plasma concentrations and all pharmacokinetic parameters of the two drugs at ALT as compared to SEA. There was a significant increase in C max and AUC 0-12h , of both drugs at ALT as compared to SEA. The values of Vd and Cl T were significantly lower at ALT than SEA. ALT could markedly inhibit the metabolism and renal excretion of both drugs as indicated by significant increase in the elimination half-lives (t 0.5e ) and decrease in the elimination rate constant (K e ) at ALT as compared to SEA. The obtained results clearly indicate that the plasma concentrations and all the pharmacokinetic parameters of both diclofenac sodium and cefadroxil are significantly modified by ALT. Therefore, dosage regiemens adjustment is important when diclofenac sodium and cefadroxil are prescribed for administration at ALT to maintain drug efficacy and safety and to avoid drug , s toxicity.

A rapid and simple high performance liquid chromatography method was developed and validated at λ=275 nm for simultaneous determination of methocarbamol, paracetamol and diclofenac sodium (lamotrigine as an internal standard) from pharmaceutical preparation. The separation was performed using methanol: water: GAA, in the ratio of 400:600:05 (v/v) as the mobile phase. The detector response was linear in the range of 5 to 45 μg, 3.25 to 29.25 μg, 0.5 to 4.5 μg for methocarbamol, paracetamol and diclofenac sodium respectively. The percentage assay of methocarbamol, paracetamol and diclofenac sodium was found between 100, 99.76 and 99.31% respectively. The described method has the advantage of being rapid and easy hence it can be applied for routine quality control analysis.
