BACKGROUND: Globally, one in ten pregnant women have diabetes; out of which, 90% contribute to gestational diabetes mellitus (GDM). Medical Nutrition Therapy (MNT) is the cornerstone for GDM treatment yet adherence to MNT among the masses is not adequately monitored as part of the routine antenatal services. The study aimed to estimate the proportion of adherence to MNT and determine the factors related toadherence among antenatal women with GDM. This study also explores the facilitators, barriers, and possible suggestions for improving adherence. MATERIALS AND METHODS: This facility-based sequential explanatory mixed-method study was conducted among 341 antenatal women with GDM at, Puducherry. The study was conducted in 2021. Dietary adherence was evaluated using Perceived Dietary Adherence Questionnaire and based on the scores obtained they were selected for in-depth interviews to explore the facilitators and barriers. Collected data wereanalysed by Chi-square test using STATA version 16. RESULTS: Out of 341 participants, the proportion of participants adherent to MNT was 135 (39.6%) with 95% CI of 34%–44%. Thepredictors for poor adherence were unemployment (PR: 0.65; 95%CI: 0.48–0.88) and good adherence was antenatal women in the 2 nd trimester (PR: 1.541; 95%CI: 1.18–2.025). Barrierstonon-adherence were financial crisis, lack of awareness ofthe need for MNT, and joint family pressure. CONCLUSIONS: About two-thirds of antenatal women with GDM are non-adherent to MNT. Unemployment and period of gestation were found to be theirdeterminants. Appropriate action has to be implemented for improving the adherence rate.

Background The beneficial effect of the dietary practice is significant reduction in the risk of developing diabetes related complication. Dietary practice among type 2 diabetes is not well-implemented in Ethiopia. Up to now, in the nation, several primary observational studies have been done on dietary adherence level and its determinants among type 2 diabetes. However, a comprehensive review that would have a lot of strong evidence for designing intervention is lacking. So, this review with a meta-analysis was conducted to bridge this gap. Methods A systematic review of an observational study is conducted following the PRISMA checklist. Three reviewers have been searched and extracted from the World Health Organization- Hinari portal (SCOPUS, African Index Medicus, and African Journals Online databases), PubMed, Google Scholar and EMBASE. Articles’ quality was assessed using the Newcastle-Ottawa Scale by two independent reviewers, and only studies with low and moderate risk were included in the final analysis. The review presented the pooled proportion dietary adherence among type2 diabetes and the odds ratios of risk factors favor to dietary adherence after checking for heterogeneity and publication bias. The review has been registered in PROSPERO with protocol number CRD42020149475. Results We included 19 primary studies (with 6, 308 participants) in this meta-analysis. The pooled proportion of dietary adherence in the type 2 diabetes population was 41.05% (95% CI: 34.86–47.24, I2 = 93.1%). Educational level (Pooled Odds Ratio (POR): 3.29; 95%CI: 1.41–5.16; I2 = 91.1%), monthly income (POR: 2.50; 95%CI: 1.41–3.52; I2 = 0.0%), and who had dietary knowledge (POR: 2.19; 95%CI: 1.59–2.79; I2 = 0.0%) were statistically significant factors of dietary adherence. Conclusion The overall pooled proportion of dietary adherence among type 2 diabetes in Ethiopia was below half. Further works would be needed to improve dietary adherence in the type 2 diabetes population. So, factors that were identified might help to revise the plan set by the country, and further research might be required to health facility fidelity and dietary education according to diabetes recommended dietary guideline.

Introdução: O confinamento gerado pela pandemia do COVID-19 causou mudanças no comportamento e consumo alimentar dos indivíduos. Em pacientes com diabetes mellitus tipo 2 (DM2) essas alterações requerem investigações. O objetivo desta revisão sistemática foi avaliar o consumo alimentar em pacientes com DM2 durante o confinamento da pandemia de COVID-19 e identificar as mudanças que ocorreram neste período.Métodos: Para a busca foi utilizada a base de dados PubMed (MEDLINE) na data 04 de fevereiro de 2022 utilizando a estratégia: (diabetes mellitus OR diabetes) AND (pandemic OR COVID-19 OR SARS-Cov-2) AND (diet OR eating OR feeding behavior). Foram incluídos artigos originais observacionais, publicados a partir do ano 2020 sem restrição de idioma, que reportassem os efeitos do bloqueio da pandemia do COVID-19 na alteração na alimentação em pacientes com DM2. A seleção dos artigos e extração de dados foi realizada por dois revisores independentes. Resultados:A busca inicial resultou em 349 estudos. Após aplicação dos critérios de elegibilidade, foram incluídos 10 estudos, totalizando 1.917 indivíduos. Houve resultados positivos como aumento do consumo de frutas e vegetais e consumo de comida caseira, e negativos como no aumento no consumo de alimentos açucarados, ultraprocessados e lanches não saudáveis. Conclusão:Houve mudanças mistas no consumo alimentar dos participantes com DM2 durante a pandemia.
