Determinants of blood pressure control in rural KwaZulu-Natal, South Africa

Abstract: Background: Hypertension is a common problem in South Africa and is known to be inadequately treated and poorly controlled. Objectives: The objective of the study was to investigate what proportion of patients prescribed antihypertensive medication had controlled blood pressure (BP) and to identify factors associated with poor control. “…Surprisingly whereas others have found significant associations between knowledge of hypertension and hypertension control [28,29], no significant association between knowledge and hypertension control was observed in this study. These findings are similar to other studies conducted in Southern Africa [27,30]. It is plausible that hypertension control levels among Non-prescribed medication refers to any drug the patient is currently taking that is not prescribed by the attending physician c Fishers' exact value participants were low due to the poor knowledge of hypertension reported.…”

“…This finding is supported by Al-Jbour, Abu Kamel & Barhoom [30] who stated that increasing patients' knowledge about the disease can achieve the goal of treatment, empower patients to make decision about their treatment, and can empower their motivation and intention to compliance with the treatment regimen. Also finding go in line with Duncan, Howe L, Manakusa & Purdy [31] who also found that Providing information about the disease and treatment regimen appears to be sufficient for compliance, and non-compliance is often attributed to inadequate knowledge.…”

“…In South Africa (SA), it is estimated that a quarter of the population between the ages of 15 and 64 suffer from hypertension, which equates to over 6.2 million people if a BP measurement of ≥ 140/90 mm Hg is used as indicative of hypertension. 5 Studies on hypertension in SA have found suboptimal BP control among hypertension patients in both rural and urban settings, 6 , 7 , 8 , 9 , 10 , 11 suggesting a vast room for improvement in the diagnosis and management of hypertension. 4 …”

“… 8 Reasons for poor control of hypertension are known to be complex and include factors such as patients’ low level of education about hypertension, non-adherence, physician inertia to treat to targets, health system factors, lack of antihypertensive medications and long distances to health facilities, all of which have been reported in many African countries. 4 , 6 , 10 , 15 , 16 , 17 …”
