The prevalence of colonization by Pneumocystis jirovecii (P. jirovecii) has not been studied in Mexico. We aimed to determine the prevalence of colonization by P. jirovecii using molecular detection in a population of Mexican patients with chronic obstructive pulmonary disease (COPD) and describe their clinical and sociodemographic profiles. We enrolled patients discharged from our hospital diagnosed with COPD and without pneumonia (n = 15). The primary outcome of this study was P. jirovecii colonization at the time of discharge, as detected by nested polymerase chain reaction (PCR) of oropharyngeal wash samples. The calculated prevalence of colonization for our study group was 26.66%. There were no statistically significant differences between COPD patients with and without colonization in our groups. Colonization of P. jirovecii in patients with COPD is frequent in the Mexican population; the clinical significance, if any, remains to be determined. Oropharyngeal wash and nested PCR are excellent cost-effective options to simplify sample collection and detection in developing countries and can be used for further studies.

Pneumocystis jirovecii, özellikle bağışıklık sistemi baskılanmış bireylerde önemli pnömoni etkenleri arasındadır. Çalışmamızda, pnömoni etiyolojisi açısından gönderilen bronkoalveolar lavaj örneklerinin Pneumocystis pnömonisi (PCP) açısından değerlendirilmesi ve altta yatan diğer etkenlerin tespiti amaçlanmıştır. Yöntem: Gönderilen bronkoalveolar lavaj örneklerine, May Grünwald/Giemsa, modifiye toluidin mavisi, immünfloresan antikor boyama ve gerçek zamanlı polimeraz zincir reaksiyonu yöntemi uygulanarak bu yöntemlerin Pneumocystis pnömonisi tanısındaki yeri araştırıldı. Bakteri, mantar etkenleriyle Mycobacterium tuberculosis ve Cytomegalovirus'ün belirlenmesi için de uygun yöntemler kullanıldı. Bulgular: Yaşları 18-90 (ortalama 53.02 yaş) arasında değişen 32'si kadın, 68'i erkek olmak üzere 100 hasta çalışmaya dâhil edildi. Hastaların 17'sine herhangi bir altta yatan hastalık saptanamamış olup, bu hastalara yalnızca pnömoni ön tanısı ile bronkoskopi yapıldı. Yalnızca bir hastada gerçek zamanlı polimeraz zincir reaksiyonu yöntemi ile Pneumocystis pnömonisi pozitifliği saptandı. Tüm hastalar değerlendirildiğinde %33'ünde bir enfeksiyon etkeni saptandığı görüldü. Sonuç: Sonuç olarak bu çalışmada, boyama yöntemleri ve gerçek zamanlı PCR ile PCP oranı düşük bulunmuştur. Daha fazla hastalarla yapılacak çalışmalarda daha yüksek oranlar elde edilebilir. Bununla beraber, bronkoalveolar lavaj örnekleri genel durumu bozuk hastalarda etiyolojiye ulaşma açısından önemidir.

Background: Histoplasma capsulatum and Pneumocystis jirovecii are respiratory fungal pathogens that principally cause pulmonary disease. Coinfection with both pathogens is scarcely reported. This study detected this coinfection using specific molecular methods for each fungus in the bronchoalveolar lavage (BAL) of patients from a tertiary care hospital. Materials and methods: BAL samples from 289 hospitalized patients were screened by PCR with specific markers for H. capsulatum (Hcp100) and P. jirovecii (mtLSUrRNA and mtSSUrRNA). The presence of these pathogens was confirmed by the generated sequences for each marker. The clinical and laboratory data for the patients were analyzed using statistical software. Results: The PCR findings separated three groups of patients, where the first was represented by 60 (20.8%) histoplasmosis patients, the second by 45 (15.6%) patients with pneumocystosis, and the last group by 12 (4.2%) patients with coinfection. High similarity among the generated sequences of each species was demonstrated by BLASTn and neighbor-joining algorithms. The estimated prevalence of H. capsulatum and P. jirovecii coinfection was higher in HIV patients.
