Detection of Functional Homotopy in Traumatic Axonal Injury

Abstract: • Traumatic axonal injury is associated with decreased interhemispheric connectivity • Traumatic axonal injury couples with widely disrupted functional connectivity • These alterations support the default, salience, integrative, and executive functions.

“…Additional conventional T2weighted (TR/TE = 4000/113 ms, slice thickness = 5 mm, matrix = 320 £ 256), T2-FLAIR images (TR/TE/TI = 8000/ 79/2722 ms, slice thickness = 5 mm, matrix = 320 £ 256) and SWI (TR/TE = 28/20 ms, flip angle = 15°, slice thickness = 1.2mm, matrix = 320 £ 256) and DWI (TR/ TE = 3100/91 ms, b = 1000 s/mm 2 , matrix =192£ 192) sequences of the brain were acquired for the diagnosis of DAI. Part of these typical lesions in DAI patients were reported in our previous work (11).…”

“…In recent years, increasing numbers of studies have revealed compensatory activation (7) or engagement (8) in working memory performance task, disrupted regional (9) and long-distance connectivity (10,11), and even network dysconnectivity (12). However, these studies relied on the assumption of the 'static' temporal strength or inter-region relationship in temporal pattern, neglecting to investigate the dynamic brain alterations or time-varying of the blood oxygen level dependent (BOLD) signal in resting-state scanning (13).…”

“…This approach differs from analyses of the regional activity intensity (amplitude of low-frequency fluctuations12), regional connectivity,13,14 and global connectivity,15 because the VMHC index emphasizes the idea that specific patterns of interhemispheric disconnection can reflect functional consequences for interhemispheric communication or coordination associated with pathological (disease) and physiological conditions 1618. In several studies, abnormal homotopic connectivity in subjects with pathological states has been reported in patients with traumatic axonal injury,17 schizophrenia,19 autism,20 and cocaine addiction 21…”

“…Eighteen DAI patients were recruited from the Emergency and Trauma Center at the First Affiliated Hospital, Nanchang University. The following DAI criteria were used as in our previous study in which the homotopic functional connectivity results were reported 10 . The following inclusion criteria were applied: (a) the patient had a closed-head TBI (involving acceleration-deceleration or high-velocity rotational forces); (b) the patient was hemodynamically stable (to ensure that transfer to the scanner was clinically safe); (c) the patient was 16–60 years of age; (d) the patient’s Glasgow coma scale (GCS) score within 24 h after trauma was more than 8 (mild-to-moderate traumatic DAI); and (e) the patient undergoes MRI scanning within 6 months after trauma occurring 1 , 15 .…”

“…Resting-state functional MRI (rfMRI) has also provided considerable information on disrupted functional homotopy 10 , long-distance connectivity 11 , and networks 12 after traumatic brain injury. Recently, a summary systemically reviewed the association between local morphometry and local intrinsic activity measures 13 .…”
