Detection of focal liver lesions in cirrhotic liver using contrast-enhanced ultrasound

Abstract: Patients with liver cirrhosis are at increased risk of hepatocellular carcinoma (HCC). Conventional or baseline ultrasound (BUS) is often used as the first-line tool for HCC surveillance or detection, but the accuracy of BUS in HCC detection or differentiation from other focal liver lesions (FLLs) is limited. Contrast-enhanced ultrasound (CEUS) represents a recent revolution in the field of ultrasonography and it has become increasingly important in the detection and evaluation of FLLs. In CEUS, HCC typically … Show more

“…It is believed that the contrast is uptaken by Kupffer cells or remains in the hepatic sinusoids in the late phase. There are new contrast agents that provide the additional post-vascular or Kupffer phase that allows an assumption of the degree of malignancy based on Kupffer function [8] . The portal and late phase enhancement can give important information about the lesion's behavior.…”

“…Several techniques have been devised to overcome the limitations of B-mode US, and CEUS can serve as a good rescue method. In one report, sensitivity and overall accuracy of detection of HCCs smaller than 2 cm improved with CEUS from 29% to 80% and 64% to 87%, respectively (21).…”
