Detection and Characterization of Intracardiac Thrombi on MR Imaging

Abstract: Contrast-enhanced inversion recovery turbo FLASH sequences were superior to dark-blood-prepared HASTE and trueFISP cine MR images in revealing intracardiac thrombi. Compared with transthoracic echocardiography, MR imaging was more sensitive for the detection of left ventricular thrombi. The characterization of thrombi may be used to predict the risk of embolism, which is higher for subacute clots than for organized thrombi.

“…Finally, MR imaging offers many more techniques for tissue differentiation that were not integrated in our study protocol to keep the overall acquisition time within certain limits. MR imaging techniques such as rest perfusion imaging or postcontrast T1w imaging with long inversion times are often used for thrombus discrimination (20), whereas semiquantitative techniques such as diffusion-weighted imaging and T1 and T2 mapping might further improve the diagnostic accuracy in the assessment of cardiac tumors.…”

“…The strength of 2DE lies in its low cost and real-time visualisation of thrombus. Although MRI can detect intracardiac thrombi with sensitivity close to that of TEE [16], its high cost is often prohibitive and expertise is often lacking. Detection of atrial lesions on CT has also been described [17][18][19].…”

“…Além disso, em massas ventriculares, a RMC também permite a diferenciação de tumores com trombos cavitários com melhor sensibilidade que o ecocardiograma 51 . No caso de trombos atriais, o ecocardiograma é o exame de escolha, pois a RMC tem sensibilidade menor nestes casos, sobretudo na avaliação do apêndice atrial.…”
