Introduction: We investigated the efficacy, safety, and impact of desmopressin on quality of sleep in treating nocturnal polyuria in elderly women. Methods: We recruited 60 women over 60 years old with lower urinary tract symptoms (LUTS), including nocturia, and with nocturnal polyuria. Nocturnal polyuria was defined as nighttime urine production exceeding 33% of the 24-hour total urine volume determined by a frequency volume (FV) chart. All patients failed to respond to treatment of their underlying disease and evening fluid restriction. Desmopressin 0.1 mg was administered orally at bedtime for 12 weeks. The participants completed a series of questionnaires on the Medical Outcomes Study (MOS) sleep scale and FV chart before and after treatment. Results: The patient population had a mean age of 69.2 ± 9.4 years (range: 61-81). The mean duration of symptoms was 61.2 ± 45.1 months. Significant decreases were evident after desmopressin treatment in the number of nocturia episodes (3.63 ± 1.61 to 2.00 ± 1.13, p = 0.01), nocturnal urine volume (p = 0.01), nocturnal polyuria index (NPI) (p = 0.01), and nocturia index (NI) p = 0.01). Among the categories of the MOS sleep scale, sleep index (p = 0.003), sleep disturbance (p = 0.001), snoring (p = 0.028), and shortness of breath (p = 0.036) significantly changed, with a decreased number of nocturia episodes. Adverse events were mild. Conclusions: Desmopressin is an effective treatment for nocturnal polyuria in elderly women, where conservative treatment has failed. Sleep quality is also improved.

A Nemzetközi Kontinencia Társaság Standardizációs Bizottság által 2002-ben kiadott, 2016-ban átdolgozásra került közle-ményei, valamint a Magyar Kontinencia Társaság ajánlásai alapján (1).Nocturia: alsó húgyúti tünet, az egyén legalább egy alkalommal az alvás megszakításával felkel vizeletet üríteni. Éjszakai vizeletmennyiség: A lefekvés és felkelés között termelődő vizeletmennyiség. Ide tartozik a reggeli első ürített vizelet, azonban a lefekvést megelőző utolsó vizeletmenynyiség nem számítandó bele. Az "éjszakai " periódus a lefekvés és az alvás befejezésének szándékával történő felkelés között eltelt időtartam. Polyuria: 24 óra alatt ürített kórosan magas, 40 ml/kg/nap feletti vizeletmennyiség (pl. 80 kg tömegű egyén esetén 3200 ml-t meghaladó vizeletmennyiség). Nocturnalis polyuria: éjszaka termelődő abnormálisan magas vizeletporció. Arányát a nocturnalis polyuria indexszel (NPi) adhatjuk meg, számolási módja a következő: NPi=[éj-szakai vizeletmennyiség/24 órás vizeletmennyiség] ×100. Jellemző, hogy a 24 óra alatt termelődő vizeletmennyiség normális mértékű (kevesebb, mint 40 ml/kg/nap). Korfüg-gő érték, kóros esetben fiatal felnőtteknél (20-40 év) az éjszakai vizeletmennyiség a teljes napi vizeletmennyiség 20%-át, 65 év felettieknél 33%-át haladja meg. Enuresis nocturna: alvás során jelentkező akaratlan vizeletvesztés. Az egyén a vizeletvesztés tényére ébred. EpidemiológiaA nocturia gyakori, az életminőséget jelentősen befolyásoló panasz. Előfordulása a két nemet tekintve hasonló, a gyakoriság az életkor előrehaladtával növekszik. Epidemiológiai köz-lemények elemzései alapján a fiatal felnőttek 2-18%-ánál, 70 éves kor felett az emberek 28-62%-ánál jelentkezik egynél többször éjszakai vizelés. Európai viszonylatban annak ellenére, hogy az 50 évnél idősebbek több mint felénél jelentkezik nocturia, orvosi konzultáció kevesebb, mint az esetek harmadában történik (2). Magyarországon ennél szerényebb adat feltételezhető. Multi fak to riá -lis kóreredetű zavar, amely hátterében leggyakrabban nocturná lis polyuria, globális polyuria, alsó húgyúti funkciózavarok vagy primer alvászavarok állnak. Ebből a változatos kóreredet-ből adódik, hogy a nocturia kivizsgálása és kezelése is alapvetően multidiszciplináris feladat. Sažetak. Benigna hiperplazija prostate (BPH) karakterizirana je umnažanjem žljezdanih i/ili stromalnih elemenata prostate. Ona može dovesti do uvećanja prostate i uzrokovati smetnje mokrenja donjeg dijela mokraćnog sustava (LUTS). Rast prostate ovisan je o dobi i utjecaju spolnih hormona, prvenstveno dihidrotestosterona. Simptomi se dijele na iritativne, opstruktivne i postmikcijske. U osnovnu obradu pacijenata spada anamneza, korištenje upitnika o smetnjama mokrenja, dnevnik mokrenja, fizikalni pregled, analiza urina, serumska koncentracija kreatinina i prostata specifičnog antigena, mikciometrija i mjerenje rezidualnog urina. Terapiju se odabire na osnovi smetnji koje pacijent ima i rezultata dijagnostičke obrade. Prvi korak u liječenju predstavljaju određene promjene u ponašanju i prehrani. U pacijenata s blagim smetnjama preporučuje se aktivno praćenje. U ostalih pacijenata započinje se s medikamentoznom terapijom. Mogućnosti medikamentozne terapije su mnogobrojne: blokatori alfa adrenergičkih receptora, inhibitori 5-alfa-reduktaze, antagonisti muskarinskih receptora, analog vazopresina -dezmopresin, agonisti beta-3 adrenergičkih receptora, inhibitori 5-fosfodiesteraze, fitoterapija i kombinirana terapija. U slučaju neuspjeha medikamentozne terapije ili prisutnosti komplikacija BPH-a preporučuje se kirurško liječenje.Ključne riječi: benigna hiperplazija prostate; medikamentozna terapija; simptomi donjeg mokraćnog sustava Abstract. Benign prostatic hyperplasia (BPH) is characterized with proliferation of glandular and stromal components of the prostate. BPH can produce prostate enlargement and caused lower urinary tract symptoms (LUTS). The growth of the prostate depends on patient's age and influence of sexual hormones, specially dihydrotestosterone. Symptoms can be divided as irritative, obstructive and postmicturation symptoms. The basic evaluation included medical history, symptom score questionnaire, bladder diary, physical examination, urinalysis, creatinine and prostate specific antigen serum concentration, uroflowmetry and postvoid residual urine. The modality of treatment depends about patients symptoms and results of diagnostic evaluation. The initial step in the treatment are behavioural and dietary modifications. In the patients with mild symptoms watchful waiting is first line treatment. In the other patients medicamentous treatment is recommended. Pharmacological management included: alpha adrenoreceptor antagonists, 5-alpha-reductase inhibitors, muscarin receptors antagonists, vasopressin analoguedesmopressin, beta-3 agonists, phosphodiesterase 5 inhibitors, phytotherapy and combination therapy. If the medicamentous therapy failed or the complications of BPH are present surgical treatment is recommended.
