Background: Skin prick test (SPT) is indicated in type I hypersensitivity reactions (rapid type), among which is Atopic Dermatitis (AD). However, the results of SPT for the tested allergens are not always accompanied by history of known allergens in AD patients. Purpose: To identify the characteristics of AD patients who underwent SPT. Methods: This was a descriptive retrospective study to determine the demographics, allergy history, SPT results, and positive and negative clinical relevance in AD patients treated at Allergy Immunology Division of Dermatology and Venerology Outpatient Clinic Dr. Soetomo General Academic Hospital Surabaya-Indonesia from January 2017 to December 2019. Results: A total of 284 AD patients were included. The adult age group (20-59 years) predominated the AD patient’s visit every year with 58 patients (60.42%), 47 patients (46.08%), and 65 patients (75.58%) in 2017, 2018, and 2019, respectively. Only 60 AD patients underwent SPT, where most patients were allergic to dust with 30 patients (50.0%). House dust mite (HDM) allergen had the highest percentage of positive SPT result with 41 patients (63.3%). The greatest clinical relevance was found in cat fur allergen, where all patients (100.0%) had clinical relevance to their SPT result. Conclusion: This study shows that most AD patients were in adult age group and had a history of dust allergy. The highest percentage of positive SPT result was found in HDM allergen and the greatest clinical relevance was found in cat fur allergen.

Background: A skin prick test is one method to detect allergies. Skin prick testing in dermatology can be performed on atopic dermatitis and urticaria. Skin prick test can be done easily, it’s relatively safe, fast, and can use many allergens simultaneously in one test. Purpose: To describe the characteristics of the skin prick test results at the clinic of Dermatology and Venereology, Mohammad Hoesin Palembang General Hospital, for the period 2016-2021. Methods: Descriptive retrospective study using a cross-sectional approach. Medical record data for the period January 2016-December 2021. Result: The number of patients who underwent skin prick testing during the period January 2016 to December 2021 was 116 patients, dominated by women (59.5%) and those aged 26-35 years (40.5%). The majority of diagnoses examined were atopic dermatitis (48.3%) and chronic urticaria (31.9%). The most positive results were for allergens in the form of house dust mites, shrimp, and crabs. The positive proportion of milkfish allergen was higher in women, while the positive proportion of shrimp allergen was higher in men. Chicken feather allergen showed significant differences between the diagnoses of atopic dermatitis, chronic urticaria and acute urticaria. Conclusion: House dust mites were the dominant allergen in patients with atopic dermatitis, chronic urticaria, and acute urticaria. skin prick test is recommended to be performed in the management of atopic dermatitis, chronic urticaria, and acute urticaria in order to detect the allergen because it has signiﬁcant clinical relevance.

Alergi pangan ialah suatu reaksi hipersensitivitas melalui mekanisme imunologis setelah terpapar alergen dari suatu bahan pangan. Arginin kinase dengan kode alergen (Pen m 2) merupakan alergen yang terdapat di bahan pangan udang windu (Penaeus monodon) dan dapat memicu terjadinya alergi. Tujuan dari penelitian ini untuk mendapatkan struktur tiga dimensi dan mendapatkan profil hidrolisat arginin kinase secara in silico. Metode penelitian dengan tahap pertama ialah pencarian identitas dan karakteristik fisikokimia arginine kinase berupa family, berat molekul, nilai pI teoritis, nilai grand average of hidropathicity (GRAVY) dan indeks kestabilan. Selanjutnya tahap kedua ialah penentuan struktur tiga dimensi. Tahap ketiga ialah memprediksi profil hasil hidrolisis protein alergen arginin kinase secara enzimatis (pepsin, tripsin dan kimotripsin) dan non-enzimatis (asam format). Hasil dari penelitian ini menjelaskan bahwa Pen m 2 merupakan family ATP:guanido phosphotransferase dengan pola kinase fosfagen. Struktur tiga dimensi Arginin kinase (Pen m 2) memiliki hasil identitas sekuens 96,91% dan nilai Ramachandran favoured ialah 97,18%. Hidrolisis arginin kinase menggunakan enzim pepsin, kimotripsin dan tripsin menghasilkan peptida terpanjang 11 asam amino (berat molekul 1210 Da), sehingga menunjukkan penurunan alergenisitas arginin kinase (Pen m 2). Hidrolisis menggunakan asam format menghasilkan peptida terpanjang ialah 69 asam amino (berat molekul 7590 Da) yang masih berpotensi alergenik Kata kunci: arginin kinase, in silico, pemotongan peptida, SWISS-MODEL, udang ABSTRACT: Food allergy is a hypersensitivity reaction through an immunological mechanism after exposure to an allergen from a food ingredient. Arginine kinase with an allergen code (Pen m 2) is an allergen found in tiger prawns (Penaeus monodon) and can trigger allergies. This study aimed to obtain a three-dimensional structure and an in silico profile of arginine kinase hydrolyzate. The research method in the first stage is the search for identity and physicochemical characteristics of arginine kinase in the form of family, molecular weight, theoretical pI value, and grand average of hydropathicity (GRAVY) and stability index. The second stage is the determination of the three-dimensional structure. The third step is to predict the profile of the hydrolysis of the allergen arginine kinase protein enzymatically (pepsin, trypsin and chymotrypsin) and non-enzymatically (formic acid). The results of this study explain that Pen m 2 is a family of ATP:guanido phosphotransferases with a phosphagen kinase pattern. The three-dimensional structure of Arginine kinase (Pen m 2) has a sequence identity result of 96.91%, and the Ramachandran favoured value is 97.18%. Hydrolysis of arginine kinase using the enzymes pepsin, chymotrypsin and trypsin resulted in the longest peptide of 11 amino acids (molecular weight 1210 Da), thus indicating a decrease in the allergenicity of arginine kinase (Pen m 2).
