Dermoscopy of syringocystadenoma papilliferum

Abstract: Syringocystadenoma papilliferum is a benign adnexal neoplasm frequently seen in association with other adnexal tumours. We report the dermoscopic features of three cases of syringocystadenoma papilliferum developing in naevus sebaceus. Clinically the lesions were characterised by exophytic papillary structures. Dermoscopically, polymorphous vessels were the prevalent feature.

“…2 This finding seemed to be consistent with the yellow areas seen in our case. Dermoscopic findings of syringocystadenoma papilliferum that sometimes show similar clinical findings to WD, reveal a polymorphous vascular pattern on a pinkish-white background, 3,4 which is different from those of WD as we show.…”

“…The reports describing the dermoscopic features of SCAP are limited in the published literature 1, 2, 3, 4, 5. In these cases, SCAP lesions were mostly associated with nevus sebaceous2, 3, 4, 5 except 1 case of linear SCAP 1 . The dermoscopic features of SCAP described in those reports were exophytic papillary structures with central depression/erosion/crust/ulceration and vascular structures (hairpin, polymorphous, or comma vessels) on a milky red or erythematous background.…”

“…3 Unlike in our case, Lombardi and colleagues reported a yellowish hue representing the underlying nevus sebaceus. 5 Molluscum contagiosum is the closest differential of our case, dermoscopically characterized by the co-existence of orifices, polylobular yellowish-white material or uniform homogenous color in the center, vascular structures (crown, punctiform, radial and mixed flower) and white rosettes. 6 This vascular pattern is restricted to the amorphous structure, and never crosses the core in contast to syringocystadenoma papilliferum, in which vascular structures may involve the central crater, as in our case.…”
