Dermoscopic Features of Basal Cell Carcinomas: Differences in Appearance Under Non-Polarized and Polarized Light

Abstract: This study highlights differences in dermoscopic features of BCCs under three dermoscopic modalities. Shiny white lines (chrysalis and crystalline structures) and shiny white areas may be used as additional criteria to diagnose BCCs.

“…The difference between them is the ivory-white color of the entire lesion in DT, with the other dermoscopic features of BCC such as leaf-like structures and ovoid nests not present. Contrary to the first 4 cases of DT described [15,16,17,18,19], in our case, upon dermoscopic examination keratin cysts were clearly visible. This feature is very rare in BCC.…”

“…Therefore, three different modalities (PNCD, PCD and NPD) are now available in daily clinical practice. It has been reported that there were some differences among the images of these three modalities [25][26][27][28][29]. Benvenuto-Andrade et al [28] evaluated the differences among them.…”

“…The recognition of dots [26,28,29], it is reported that the vasculature was better visualized by PD than by NPD because deeper structures were better visualized by polarized light. The dermoscopic observation in this study enabled the vascular structure of the superficial dermis to be observed without interference by the epidermal layer; however, polarized light even made a difference in the visibility of vessels.…”

“…Based on the current literature the dermoscopic patterns of FeP can be summarized as follows: (1) polymorphous vessels consisting mainly of fine, focused, short arborizing and dotted vessels, the latter mainly located at the periphery of the lesion (17 cases; 85%); (2) short white streaks (15 cases; 75%), also called chrysalis/crystalline structures, are only visible using polarized dermoscopy [17,18,19,20,21], thus not recognizable in our photographs, which were taken with a nonpolarized light source; (3) additional criteria, including milia-like cysts (7 cases; 35%) and ulceration (9 cases; 45%), have been reported but are not specific of FeP [20], and (4) in cases of pigmented FeP, gray-brown areas and gray-blue dots can also be seen (8 cases; 40%) [5]. …”
