Dermatomyositis Induced by Filler Rhinoplasty Using Liquid Silicone

Abstract: rhinoplasty using liquid silicone. Furthermore, the reported median time between exposure to foreign material and the onset of symptoms of autoimmune diseases was 1 month and ranged from 2 weeks to 5 years. 2 This case demonstrates that the period between liquid silicone injection and the onset of autoimmune disease can exceed 20 years. Thus, clinicians must consider the possibility of liquid silicone-induced autoimmune disease when presented with patients reporting a history of liquid silicone injection.

“…It is important to note that identifying culprit drugs and connecting them temporally with disease course is difficult for several reasons. First, many DM patients (82)(83)(84)(85)(86) Pregnancy (87)(88)(89)(90)(91)(92)(93) Liquid silicone filler rhinoplasty (94) Silicone breast implants (95) have underlying malignancies or autoimmune disorders, predisposing them to or triggering DM (6,106). Second, patients are often on numerous drugs, and determining the offending agent can therefore be challenging.…”

“…Overall, various factors during pregnancy such as exposure to fetal antigens, changes in maternal hormonal status, and the reactivation of certain viruses by pregnancy have been considered as triggers for the development of DM (93). Other miscellaneous triggers of DM include liquid silicone filler rhinoplasty and silicone breast implants (94,95).…”

“…The porosity of ePTFE serves as an anchor; however, it also poses a serious challenge when removal is necessary. Unlike silicone implants (which can be easily removed), ePTFE-coated silicone implants can be difficult to remove securely and completely using a closed approach. Zelken et al recommended an open approach for composite implant removal.…”

“…Tumor necrosis factor alpha (TNF-a) has also been implicated in the pathogenesis of DM [11]. Environmental factors, such as changing season, ultraviolet light exposure, drugs (hydroxyurea, penicillamine, statin drugs, quinidine and phenylbutazone), infectious agents such as Toxoplasma, Borrelia, coxsackie virus, parvovirus, echovirus, human T-cell lymphotropic virus (HTLV-1), Human immunodeficiency virus (HIV), Hepatitis B and C viruses, filler materials (silicone) [12] and certain lifestyle factors have been suggested as triggering factors [13].…”
