Depression, posttraumatic stress, and alcohol misuse in young adult veterans: The transdiagnostic role of distress tolerance

Abstract: Background Alcohol misuse is common among young adult veterans, and is commonly associated with depression and posttraumatic stress disorder (PTSD). In fact, rates of comorbid depression, PTSD, and problem drinking are high in this population. Although distress tolerance, the capacity to experience and withstand negative psychological states, has been examined as a potential transdiagnostic factor that accounts for the development of mental health disorders, problem drinking, and the comorbidity between these … Show more

“…With each additional PTE, distress tolerance may be lowered which further decreases an individual's capacity to cope effectively (Fetzner et al 2014), and in turn may increase the risk of experiencing additional PTEs (Finkelhor et al 2009). Research has found that distress tolerance difficulties mediate the relation between PTEs and PTSD severity and other co-occurring conditions such as alcohol use (Holliday et al 2016;Vujanovic et al 2011b) and suicidal attempts (Anestis et al 2012); these in turn may be a risk factor for additional PTEs (Ehrensaft et al 2003;Finkelhor et al 2009). …”

“…Ongoing effects of combat related PTSD can include increased alcohol and drug use (Holliday, Pedersen, & Leventhal, 2016), participating in risk taking behaviours (American Psychiatric Association, 2013), increased risk of interpersonal violence (Forbes & Bryant, 2013) and psychosis (Richardson et al, 2010).…”

“…A parsimonious explanation of this comorbidity may be that a small set of transdiagnostic psychopathologic vulnerabilities that give rise to numerous mental health conditions may also contribute to and result from marijuana use [7]. Such transdiagnostic vulnerabilities may account for the pervasive patterns of psychiatric comorbidity with use of marijuana and other substances [8–10]. …”
