Objective: to verify the association between the health-related quality of life of chronic renal patients on hemodialysis with sociodemographic, clinical, depression and medication adherence characteristics. Method: a cross-sectional study with 183 chronic renal patients undergoing hemodialysis in the state of Rio Grande do Sul, Brazil. A sociodemographic and clinical questionnaire, Kidney Disease and Quality of Life Short-Form, Beck Depression Inventory and Morisky Medication Adherence Scale - eight items were used. Among the variables, comorbidities, complications of kidney disease and intercurrences during and after hemodialysis were evaluated. The analysis was performed with descriptive and analytical statistics. Results: 55.2% of the patients were 60 years old or older, 35.0% were hypertensive, with regular quality of life, average of 62.61. Scores below average in the dimensions of quality of life were mainly associated with repetitive infections and edema as complications of the disease, pain during hemodialysis and weakness afterwards. Low drug adherence resulted in a worse quality of life, impacting ten of the 20 dimensions evaluated and depression in all, except for patient satisfaction. Conclusion: reduced quality of life in this population is associated with depressive symptoms, complications such as repetitive infections, pain and anemia, weakness after the dialysis session and low medication adherence. Actions aimed at changing these factors can promote well-being.

Chronic kidney disease (CKD) is characterized by kidney damage with proteinuria, hematuria, and progressive loss of kidney function. The final stage of CKD is known as end-stage renal disease, which usually indicates that approximately 90% of normal renal function is lost, and necessitates renal replacement therapy for survival. The most widespread renal replacement therapy is dialysis, which includes peritoneal dialysis (PD) and hemodialysis (HD). However, despite the development of novel medical instruments and agents, both dialysis procedures have complications and disadvantages, such as cardiovascular disease due to excessive blood fluid and infections caused by impaired immunity. Periodontal disease is chronic inflammation induced by various pathogens and its frequency and severity in patients undergoing dialysis are higher compared to those in healthy individuals. Therefore, several investigators have paid special attention to the impact of periodontal disease on inflammation-, nutrient-, and bone metabolism-related markers; the immune system; and complications in patients undergoing dialysis. Furthermore, the influence of diabetes on the prevalence and severity of manifestations of periodontal disease, and the properties of saliva in HD patients with periodontitis have been reported. Conversely, there are few reviews discussing periodontal disease in patients with dialysis. In this review, we discuss the available studies and review the pathological roles and clinical significance of periodontal disease in patients receiving PD or HD. In addition, this review underlines the importance of oral health and adequate periodontal treatment to maintain quality of life and prolong survival in these patients.

AbstrakPasien End Stage Renal Disesase (ESRD) yang menjalani hemodialisis rutin, masih mengalami masalah pada fungsi fisik, psikososial dan kualitas hidup. Aktivitas fisik yang rendah telah dikaitkan menjadi penyebab rendahnya kualitas hidup pada pasien hemodialisis. Tujuan: Menyelidiki dampak Intradialytic Range Of Motion (ROM) exercise terhadap depresi, insomnia dan asupan nutrisi pada pasien hemodialisis. Metode: Penelitian ini merupakan studi eksperimen dengan desain pre and post test with control group. Ada 23 pasien pada kelompok intervensi dan 24 pasien pada kelompok kontrol yang diambil menggunakan simple random sampling. Intervensi berupa intradialytic ROM exercise yang melibatkan gerakan aktif dari ekstremitas dilakukan selama tiga bulan, dua kali seminggu selama tiga puluh menit dengan intensitas latihan 12 sampai 14 pada skala Borg's Rating of Perceived Exertion (RPE). Hasil:Latihan memberikan dampak positif terhadap rerata skor Beck Depression Inventory (BDI) yang signifikan menurun (p < 0,001) dan juga terhadap skor Insomnia Severity Index (ISI) (p < 0,05), sedangkan terhadap asupan kalori dan protein tidak mengalami perubahan yang signifikan (p > 0,05). Simpulan: Intradialytic exercise dengan intensitas sedang sangat direkomendasikan untuk pasien yang sedang menjalani hemodialisis karena dapat menurunkan depresi dan insomnia sehingga meningkatkan kualitas hidup pasien yang menjalani hemodialisis. Kata kunci: intradialytic exercise, gerakan aktif, depresi, insomnia, asupan nutrisi Abstract The patients of end stage renal disease (ESRD) who undergo routine hemodialysis still experince problems with poor physical, psychosocial and quality of life. Low physical activity has been linked to the cause of low quality of life in hemodialysis patients. Objectives: to investigated the effect of intradialytic Range of Motion (ROM) exercise on depression, insomnia and nutritional intake in hemodialysis patients. Methods: This study was an experimental study with pre and posttest with control group design. There were twenty three patients in the intervention group and twenty four patients were taken by simple random sampling. The intervention in this study was in the form of ROM exercises involving active movements of the extremities was conducted for three months, twice a week for thirty minutes while undergoing hemodialysis with the intensity of exercises 12-14 on the Borg's scale Rating of Perceived Exertion (RPE).Results: Exercise had a positive impact on the average Beck Depression Inventory (BDI) score which significantly decreased (p < 0.001) and also on the Insomnia Severity Index (ISI) score (p < 0.05), while the calorie and protein intake did not change significantly (p > 0.05). Conclusions: The intradialytic exercise with medium intensity was highly recommended for patients who were underwent hemodialysis because it could reduce depression and insomnia thereby improving the quality of life of patients undergoing hemodialysis.
