Definition and significance of high positivity aCL ELISA

Abstract: We conducted a prospective study of anticardiolipin antibody (aCL) testing, performed in our university hospital. Among 6321 consecutive patients tested for both IgG and IgM aCL, 91 patients with medium or high positivity (>99th percentile) had a subsequent confirmatory test. Among them, 53 had a persistent positivity at 12 weeks. In this real world setting, patients with transient positivity had lower values than patients with persistent positivity. “…Patients found LA‐positive, and those with high IgG aCL titres or with high LA ratios (which can be considered as result of a high titre as well) at initial testing, were significantly more frequent in the EPP than in the NPP group. This extends existing reports concerning confirmation at 12 weeks, showing that the presence of a LA is associated with aPL persistence at that time point and that patients with transient aPL positivity have lower aCL titres than patients with confirmed aPL positivity …”

“…IgG and IgM aβ 2 GPI ELISAs were performed with a commercially available kit (Orgentec ® ). For both ELISAs, the calibration was performed according to Harris's standards and the thresholds for positivity were defined as the 99th percentile of the titre measured in a population of more than 170 healthy blood donors . The cut‐off values for aCL positivity were equal to 14 GPL and 5 MPL for IgG and IgM, respectively, and those for aβ 2 GPI positivity were equal to 8 GPL and 8 MPL for both IgG and IgM.…”

“…Unlike triple‐positive and double‐positive subjects, single‐positive subjects had laboratory data confirmed after 12 weeks in 40% of cases. A similar figure (53 of 91 had persistent positivity at 12 weeks) was recently found in patients with medium/high aCL positivity (> 99th percentile) . However, subjects with persistent aCL had higher values than subjects with transient aCL, the latter being close to medium‐positivity thresholds .…”
