ObjectiveTo understand patient engagement with decision-making for infection management in secondary care and the consequences associated with current practices.DesignA qualitative investigation using in-depth focus groups.ParticipantsFourteen members of the public who had received antimicrobials from secondary care in the preceding 12 months in the UK were identified for recruitment. Ten agreed to participate. All participants had experience of infection management in secondary care pathways across a variety of South-East England healthcare institutes. Study findings were subsequently tested through follow-up focus groups with 20 newly recruited citizens.ResultsParticipants reported feelings of disempowerment during episodes of infection in secondary care. Information is communicated in a unilateral manner with individuals ‘told’ that they have an infection and will receive an antimicrobial (often unnamed), leading to loss of ownership, frustration, anxiety and ultimately distancing them from engaging with decision-making. This poor communication drives individuals to seek information from alternative sources, including online, which is associated with concerns over reliability and individualisation. Failures in communication and information provision by clinicians in secondary care influence individuals’ future ideas about infections and their management. This alters their future actions towards antimicrobials and can drive prescription non-adherence and loss to follow-up.ConclusionsCurrent infection management and antimicrobial prescribing practices in secondary care fail to engage patients with the decision-making process. Secondary care physicians must not view infection management episodes as discrete events, but as cumulative experiences which have the potential to shape future patient behaviour and understanding of antimicrobial use.

RESUMOObjetivo: analisar estudos qualitativos sobre percepções de pacientes relativos às medidas para prevenção e controle das infecções relacionadas à assistência à saúde e fatores que contribuem para a própria segurança. Método: revisão sistemática de publicações do período entre 2006 e 2016, seguida por síntese temática por meio da codificação dos relatos de pacientes encontrados nos manuscritos e das interpretações dos achados. Resultados: seis temas evidenciaram barreiras para a participação ativa do paciente na promoção de sua segurança, oferta insuficiente de informações; dificuldade de compreensão; sentimentos negativos; vivências negativas; comportamento dos profissionais; e elementos que influenciam o envolvimento do paciente. Conclusão: a lacuna existente entre o entendimento das medidas de prevenção e controle de infecções pelos pacientes e seu envolvimento no processo indica a necessidade de ações que favoreçam comunicação eficaz, melhores relações profissionais-pacientes, e acesso a informações que incentivem a interação e contribuição para uma assistência segura. Method: systematic review of publications from 2006 to 2016, followed by thematic synthesis by coding patients' accounts and interpretations of findings given. Results: six themes highlighted barriers to patients' participating actively in promoting their own safety: insufficient information supply; poor comprehension; negative feelings; negative experiences; behavior of health care personnel; and factors that influence patient involvement. Conclusion: the gap between patients' understanding infection prevention and control measures and their becoming involved in the process point to a need for measures to foster effective communication, better care personnel-patient relations, and access to information that encourages patients to interact and contribute to health care. In this chapter we review the development of hospital infection prevention and control (IPC) since the nineteenth century and its increasingly important role in reducing the spread of antibiotic resistance (ABR). Excessive rates of hospital-acquired infection (HAI) fell dramatically, towards the end of the nineteenth century, because of improved hygiene and surgical antisepsis, but treatment remained rudimentary until effective antibiotics became widely available in the mid-twentieth century. While antibiotics had profound clinical benefits, their widespread appropriate and inappropriate use in humans and animals inevitably led to the emergence of antibiotic resistance (ABR). Within 50 years, this could no longer be offset by a reliable supply of new drugs, which slowed to a trickle in the 1980s. In hospitals, particularly, high rates of (often unnecessary) antibiotic use and ABR are exacerbated by person-to-person transmission of multi-drug resistant organisms (MDRO), which have, so far, largely resisted the introduction of antimicrobial stewardship (AMS) programs and repeated campaigns to improve infection prevention and control (IPC). Despite clear evidence of efficacy in research settings, both AMS and IPC programs are often ineffective, in practice, because of, inter alia, insufficient resourcing, poor implementation, lack of ongoing evaluation and failure to consult frontline staff. In this chapter we review reasons for the relatively low priority given to preventive programs despite the ethical obligation of healthcare organisations to protect current and future patients from preventable harm. The imminent threat of untreatable infections may provide an impetus for a shared organisational and professional commitment to promoting the cultural and behavioural changes needed to successfully reduce the burdens of ABR and drug-resistant HAIs.
