Deep Anterior Lamellar Keratoplasty in Patients with Keratoconus: Big-Bubble Technique

Abstract: : DALK using the big-bubble technique appears to be a safe and effective procedure for eyes with moderate to advanced keratoconus. In case of extensive intraoperative DM perforation, it does not pose any limitation to ongoing penetrating keratoplasty.

“…Postoperative BCVA scores were improved over preoperative values in all patients. These visual outcomes are comparable to results obtained in other studies using the big-bubble technique but involving the removal of the donor endothelium (15,16,23) . The incidence of graft rejection following DALK has been reported to range from 0% to 9.6% in various series (13,24,25) .…”

“…Comparative studies found no significant differences after DALK in VA and contrast sensibility, irrespective of whether the donor endothelium was removed or left attached (20)(21)(22)(23) . The present study aimed to evaluate visual outcomes in patients with keratoconus undergoing DALK using full-thickness donor endothelial grafts.…”

“…Both cases had wide anterior lamella (9.3 and 8.5 mm) to avoid suturing into ectatic corneal tissue and pre-existing corneal neovascularizations. The close proximity between the limbus and the wider anterior lamella of the mushroomshaped corneal graft was speculated to be reason behind the higher rate of stromal rejection as previously observed by Feizi et al 29 and Shehadeh-Mashor et al 14 In summary, our study showed the potential of FSL-assisted mushroom-configuration keratoplasty in the management of patients with stage IV advanced keratoconus, and the described AMID could further enhance the post-keratoplasty refractive and astigmatic outcomes. Future studies with larger multicentre cohorts would help to determine the preferred surgical technique in this particular subgroup of patients.…”
