Declines in serum free and bound choline concentrations in humans after three different types of major surgery

Abstract: We examined the changes in circulating choline status in humans in response to major surgery by measuring serum free and phospholipid-bound choline concentrations before, during and 1-72 h after total abdominal hysterectomy, off-pump coronary artery graft surgery or brain tumor surgery. Preoperatively, the mean serum free and phospholipid-bound choline concentrations in patients scheduled for abdominal hysterectomy (n = 26), off-pump coronary artery grafting surgery (n = 34) or brain tumor surgery (n = 24) wer… Show more

“…Sixty (60) of the 242 mothers (25%) had a history of anxiety disorder. Mothers with a history of anxiety disorder were more likely than mothers without to meet diagnostic criteria for current or residual anxiety (63% versus 0%; chi 2 = 136.7; p <.001) and depressive disorders (23% versus 4%; chi 2 = 16.7; p <.001) and to utilize antidepressants (24% versus 7%; chi 2 = 11.9; p =.001) during pregnancy.…”

“…1). To our knowledge, there are few data available on the relation between free and bound choline concentrations in the circulation, and the published data in dogs and humans show a decline in serum-free choline accompanied by a decrease in serum phospholipid-bound choline concentration in response surgery (13,14), childbirth (12), running a marathon (31), and methylprednisolone administration (14). Serum-free and phospholipid-bound choline increase together during pregnancy (12).…”

“…Although these data clearly demonstrate that availability of choline is very important in response to endotoxin, to our knowledge, no data is available about circulating choline status during endotoxemia. Recent studies from our laboratory have shown that serum-free and phospholipid-bound choline concentrations decline in response to surgical and traumatic injuries in humans (10)(11)(12)(13) and in dogs (14). In light of these observations, and considering that endotoxemia is associated with tissue injury and multiorgan damage/failure, we hypothesized that circulating choline status is likely to be altered during endotoxemia.…”

“…Interestingly, smoking is associated with significantly lower PCho concentrations and the reasons for this are unknown [8]. Declines and/ or lower levels of plasma or SCho have been found after major surgery [41], traumatic head injury [40] and more gradually during long-term parenteral nutrition [42]. These and other factors leading to abnormal choline levels are summarized in Box 3.…”
