Debridement of Diabetic Foot Ulcers

Abstract: Diabetic foot ulcerations have devastating complications, including amputations, poor quality of life, and life-threatening infections. Diabetic wounds can be protracted, take significant time to heal, and can recur after healing. They are costly consuming health care resources. These consequences have serious public health and clinical implications. Debridement is often used as a standard of care. Debridement consists of both nonmechanical (autolytic, enzymatic) and mechanical methods (sharp/surgical, wet to … Show more

“…Small and unmyelinated nerve fibers responsible for conducting afferent sensory perception, like C-type fibers, are the first to be damaged, resulting in tissue damage due to poor perception of trauma and/or mechanical stress. Thus, the relatively minor damage will continue to accumulate and result in a progressively worsening wound with difficulty in healing[ 33 ].…”

“…The above observations made in the course of the analysis unambiguously draw attention to the continuation of training and paying attention to the above method, which is becoming more and more appreciated in Poland, especially at the times of pandemic COVID 19/SARS-CoV-2. Taking into account the increasing number of patients with wounds, the difficulties related to the availability of hospital care, home-based and outpatient wound therapy with MDT is gaining new meaning, and may indicate new directions of clinicians’ activities when combined with the subsequent use of NPWT (Negative Pressure Wound Therapy) [ 18 , 19 , 20 , 21 ]. The 2020 guidelines for the care and treatment of pressure ulcers of the Polish Wound Treatment Society (PTLR) recommend MDT in the treatment of pressure ulcers [ 11 ].…”

“…Debridement is the process of removal of necrotic, infected tissue leaving behind healthy tissue. 46 There are various types of debridement methods such as surgery, enzymatic, biological, and autolytic debridement that can be used in wound healing. 47 In the surgical approach, scalpels are used to remove dead tissue effectively, while in the enzymatic debridement method, necrotic tissue is removed using enzymatic agents.…”
