The 2011 Great East Japan Earthquake (GEJE) has renewed the paradigm of disaster medicine. The Tohoku Journal of Experimental Medicine (TJEM) widened its scope to include the disaster science from the health perspectives. TJEM has been accumulating 76 articles related with "disaster" or "pandemic" out of which 69 were published after 2011. Tohoku University established the International Research Institute of Disaster Science (IRIDeS) that took initiative to impact the Sendai Framework for Disaster Risk Reduction 2015-2030 (Sendai Framework) to include health aspect. Sendai Framework provided the platform for collaboration of different sectors, including the terminology that defines the concept of disaster, risk, hazard & exposure, vulnerability and coping capacity. Disaster medicine covers the diverse causes and damages of disasters by various hazards in this globalized and rapidly changing world. TJEM articles range the physical and mental health damage after the GEJE and other disasters with approved ethical consideration of investigations from the view point of affected area, mechanisms of hazard to affect human health including the radiation, virus or hazardous materials, proposal of refinement of health system to cope with disasters such as mental health support, risk communication, disaster medical coordination and hospital business continuity plan and future perspectives with reconstruction including Tohoku Medical Megabank Project. TJEM scope on disaster medicine had been widened during the 10 years after GEJE and IRIDeS can be the bridging hub not only between the health sector and other sectors, but also between disaster medicine and other medical disciplines.

Homeless Tent Fires: A Descriptive Analysis of Tent Fires in the Homeless Population

Tent fires are a growing issue in regions with large homeless populations given the rise in homelessness within the US and existing data that suggest worse outcomes in this population. The aim of this study is to describe the characteristics and outcomes of tent fire burn injuries in the homeless population. A retrospective review was conducted involving two verified regional burn centers with patients admitted for tent fire burns between January 2015 and December 2020. Variables recorded include demographics, injury characteristics, hospital course, and patient outcomes. Sixty-nine patients met the study inclusion criteria. The most common mechanisms of injury were by portable stove accident, assault, and tobacco or methamphetamine-related. Median percent total body surface area (%TBSA) burned was 6% (IQR 9%). Maximum depth of injury was partial thickness in 65% (n=45) and full thickness in 35% (n=24) of patients. Burns to the upper and lower extremities were present in 87% and 54% of patients, respectively. Median hospital Length-of-Stay (LOS) was 10 days (IQR=10.5) and median ICU LOS was 1 day (IQR=5). Inhalation injury was present in 14% (n=10) of patients. Surgical intervention was required in 43% (n=30) of patients, which included excision, debridement, skin grafting, and escharotomy. In-hospital mortality occurred in 4% (n=3) of patients. Tent fire burns are severe enough to require inpatient and ICU level of care. A high proportion of injuries involved the extremities and pose significant barriers to functional recovery in this vulnerable population. Strategies to prevent these injuries are paramount.

17 Ağustos 1999 Marmara Depremi sonrasında Bolu Gölyaka ilçesinde kurulan iki çadırkentte 30 günlük izlem; Düzce’de ise araştırma süresinde kurulu olan en büyük üç çadırkentte durum saptama araştırması yapılmıştır. Gölyaka ve Düzce'de kurulan çadırkentlerde yaşamın halk sağlığı yönünden incelenmesinin ilerde benzeri durumlarda yol gösterici olması amaçlanmıştır. Gölyaka’da 88 Düzce’de 83 olmak üzere toplam 171 çadır araştırma kapsamında yer almıştır. Araştırmada bazı sosyodemografik özellikler, deprem hasarına ve sağlık durumuna ilişkin bilgiler Hane Halkı Bilgi Formu ve Kişisel İzlem Bilgi Formu, çevresel özellikler Çevre Sağlığı Bilgi Formu ile toplanmıştır. Gölyaka'da araştırmaya alınanların %51,0'i erkek, %49,0'u kadın; Düzce’de %49,7'si erkek, %50,3 'ü kadındır. Gölyaka'da araştırmaya katılan kişilerin %47,8'i ilkokul, Düzce'de ise %12,1 'i ilkokul mezunudur. Gölyaka'da çalışmaya katılanların % 31,4'ü öğrenci, %30,8'i ev hanımıdır. Düzce'de %26,7'si öğrenci, %23,6'sı ev hanımıdır. Gölyaka’da binaların %50'si, Düzce’de %26,5'i hasarlı oturulamaz durumdadır. Gölyaka'da izlenen çadırkentlerde deprem sırasında oturulan evlerin %45,5'i dört katlı, %18,2'si 3 katlı; Düzce'de %27,7'si beş katlı, %43,4'ü dört katlı binalardır. Deprem sonrasında araştırmaya katılanların yarısında iş kaybı ve gelirde azalma görülmüştür. Aile Planlaması yönünden incelendiğinde korunmama artmış, yöntem kullanımı azalmıştır. Cinsel aktivite olumsuz etkilenmiştir. İzlem yapılan çadırkentlerde de en sık öksürük-balgam, soğuk algınlığı-nezle görülmüştür. Çevre sağlığı yönünden kamp yeri ve su özellikleri olumludur. Tuvaletler temizlikleri hariç genelde olumludur. Banyo, çamaşırhane ve mutfak özellikleri yetersizdir. Atıklar yeterli sayıda çöp bidonu bulunması ve günlük olarak toplanmaları sonucunda olumsuz çevre koşulu yaratmamıştır. Gölyaka’da ortak mutfaklarda sunulan yemekler önerilen enerji ve besin ögelerini sağlamamıştır. Ancak beslenmeye ilişkin herhangi bir sorun gözlenmemiştir. Afet planlamalarında çadırkent düzenlemeleri yer almalıdır.
