Danger of sputum purulence screens in culture of Legionella species

Abstract: Previous studies have shown the benefit of sputum purulence screens in routine bacterial culture. We have correlated 944 sputum smear and LegioneUla pneumophila culture results and demonstrated that 47 to 84% of L. pneumophila-positive samples would have been discarded by using established sputum purulence screens. We recommend acceptance of all specimens submitted for Legionela culture.

“…All the collected sputum samples were cultured, as sputum from LD – patients often do not fulfill the routine microscopic criteria for representative lower airway samples . This means that some non‐representative lower airway samples might have been analysed.…”

“…Most laboratories also report an improvement in the quality of specimens once these standards are imposed. Two caveats regarding the use of these criteria are that cytologic screening is not appropriate for studies of Legionella spp [34] or acid-fast bacilli, and specimens obtained using hypertonic saline (induced sputum) generally provide inadequate specimens because of high concentrations of SECs [35].…”

“…Nonpurulent sputum with few polymorphonuclear leukocytes and numerous squamous epithelial cells are traditionally considered to be inadequate specimens for cultures; however, many such specimens have yielded L. pneumophila by culture. 25,40…”
