Men who have sex with men (MSM) are more likely to use drugs and other substances compared to their heterosexual peers. No studies have evaluated patterns of substance use among MSM adults in Mexico. We used latent class analysis (LCA) to identify MSM subgroups with specific substance use patterns and their associations with sexual behaviors. Methods: Data from 1850 adult MSM were collected at HIV clinics in Mexico City between September 2018 and December 2019. The structural equation modeling approach was used to estimate a LC model to identify patterns of substance use by self-report of substance use (i.e., cigarette smoking, alcohol, and drugs). To evaluate LC membership, we included HIV status, condomless anal sex (CAS), and serosorting, while controlling for demographic variables. Results: 30.3% were under the age of 22. Alcohol use in last 30 days (76.2%), binge drinking (29.2%), marijuana (29.4%), sex-drugs (23.9%), stimulants (13.7%), and depressants (6.3%). MSM reported engaging in CAS (55.9%) and serosorting (13.5%) behaviors, and 40% reported being HIV positive. LCA indicated three general categories of MSM substance users: Class 1 (49.0%), Class 2 (29.8%), and Class 3 (20.4%). Members of Class 3 were younger: 23–28 age years (aOR = 1.86) and 29–33 age years (aOR = 1.86), more educated: completed graduate studies (aOR = 1.60), had a high probability of polysubstance use and were more likely to engage in CAS and serosorting. Conclusions: Attempts to detect alcohol and problematic use of substances are needed for MSM followed by culturally competent approaches that address alcohol and drug use disorders.

Introducción: En Colombia, las prácticas de hombres que tienen sexo con hombres (HSH) que hacen uso de drogas sexualizadas han sido poco estudiadas, lo que dificulta el diseño de programas pertinentes para la prevención de las infecciones de transmisión sexual (ITS) y el virus de la inmunodeficiencia humana (VIH). Objetivos: Interpretar los contenidos sobre la vulnerabilidad al VIH/ITS en HSH usuarios de drogas sexualizadas en Armenia y Pereira, durante el 2022, por lo que se indagó sobre las prácticas de consumo de drogas, la percepción del riesgo y las estrategias para la reducción de riesgos y daños frente al VIH e ITS y el consumo de drogas. Métodos: En esta investigación cualitativa, se hizo un análisis de contenido a partir de veintidós entrevistas semiestructuradas hechas a HSH. Resul-tados: Los participantes han normalizado el VIH, lo que flexibiliza sus prácticas de autocuidado, implementan estra-tegias de reducción de riesgos y daños para mitigar los efectos adversos del uso de drogas, y procuran un balance entre seguridad y placer. Conclusiones: Se observaron los fenómenos conocidos como “fatiga de la prevención” y “habitus de resistencia”, así como la necesidad de desarrollar estrategias específicas de prevención en este grupo poblacional.

Pre-exposure prophylaxis and its implications in Mexico: notions of men who have sex with men

Background The aim of this study was to analyse ideas regarding pre-exposure prophylaxis (PrEP) for the prevention of HIV among groups of men who have sex with men in Mexico for future implementation in health services. Methods: During 2015, 54 people participated in four focus groups in three Mexican cities. Issues related to challenges for uses and limitations of PrEP were explored. Results: In contrast with other qualitative studies, which emphasised problems with relationships with other people for PrEP use or access to key populations, Mexican participants focused their concerns around the public health services organisation: PrEP is too expensive and the health services have no resources, which will affect services and result in discrimination. Participants identified possible stigmatisation related to prejudices of medical providers who do not approve the decreased use of condoms. As a potential solution, participants suggested that the Mexican Government could negotiate a lower cost for PrEP and public health services could provide the medication, with periodical review of the arrangements by civil organisations. Conclusion: In the current context of the new Mexican government, it is necessary to propose public politics focused on negotiating with pharmaceutical companies on the costs of the PrEP, coordinating public services with groups within civil society and strengthening policies and actions to reduce stigma and discrimination.
