Cystic Renal Cell Carcinomas in Adults. Is Preoperative Recognition of Multilocular Cystic Renal Cell Carcinoma Possible?

Abstract: Preoperative recognition of MCRCC is possible using strict computerized tomography and/or magnetic resonance imaging criteria. The current study confirms the low malignant potential of MCRCC. Nephron sparing surgery should be proposed when MCRCC is suspected.

“…Since these early theories, it is apparent that there are several permutations on the renal cysts and RCC theme that merit separation (33)(34)(35)(36)(37)(38)(39)(40). These are noted in Table 3 with comments on their significance.…”

“…mcRCCs have consistently fared better than their counterparts on both cancer-specific and overall survival. 24,[66][67][68][69][70][71][72][73] One potential explanation for this better prognosis is that the majority of mcRCCs tumour volume is fluid and thus, the actual tumour burden is much lower when compared to similar sized solid tumours. 67 As the outcomes of these tumours do not seem to be influenced by the overall lesion size, some experts have even suggested to abandon the current pathological T staging for mcRCC and to reassigned them a new stage called pT1c (c for cystic).…”

“…MCRCC must be differentiated from clear-cell RCC with cystic change, which has a worse prognosis [25]. On imaging, MCRCC is a multilocular cystic mass with regular thin cyst walls and regular septa and importantly, lacks an expansile nodule.…”
