Cyclosporine in Anti-Jo1-positive Patients with Corticosteroid-refractory Interstitial Lung Disease

Abstract: CYC is effective and substantially safe in patients with anti-Jo1 antisynthetase syndrome with corticosteroid-refractory ILD. CYC withdrawal may be associated with ILD relapse, and low-dose CYC was effective in ILD control.

“…Cyclosporin A can be prescribed as a single drug, or in combination with other immunosuppressive drugs in severe cases [58,93,97]. It seems to be effective as both a first-or second-line therapy in rapidly progressive ILD [103,106]. In chronic ILD, an efficacy of 55% has been reported when used as a first-line treatment (table 5) [86,92,97,100]; all patients responded to treatment when cyclosporin A was used as second-line in three small series [93,102,103].…”

“…In 1998, the first nation-wide survey for the treatment with CSA in IP associated with CTDs was conducted in Japan, and the efficacy of a combination therapy with CSA and corticosteroids in PM/DM associated IP was indicated [176]. A number of retrospective and open-label studies have supported the benefit of CSA for the treatment of ILD with PM/DM [177][178][179][180][181].…”
