Эстрадиол является одним из основных женских половых гормонов и играет ключевую роль в функ-ционировании репродуктивной системы. Главный источник эстрогенов в организме женщины -тека-клетки антральных фолликулов, в которых эстради-ол синтезируется из андрогенов (андростендиона и тестостерона) под действием фермента ароматазы. Основная мишень эстрогенов -органы репродук-тивной системы: матка, яичники, влагалище, маточ-ные трубы и молочные железы.Методы вспомогательной репродукции, широко применяемые в клинической практике в последние десятилетия, позволяют повысить вероятность на-ступления беременности во многом благодаря ис-пользованию препаратов, стимулирующих рост мно-жества фолликулов, вследствие чего в организме женщины существенно повышается уровень эстра-диола. При этом имплантация и развитие эмбриона происходят на фоне выраженной гиперэстрогении, иными словами, даже без назначения экзогенных препаратов эстрадиола при ВРТ имеет место гипер-эстрогения, влияние которой на эмбриогенез и здо-e-mail: a-smirnova@mail.ru ровье будущего ребенка изучено недостаточно. Это же относится к искусственной гиперэстрогении, ко-торая создается в результате широкого назначения препаратов эстрогенов многим пациентам в про-граммах ВРТ.Особое место в лечении бесплодия с помощью методов ВРТ занимает донорство ооцитов. Подго-товка эндометрия к беременности у реципиентов до-норских ооцитов с нефункционирующими или уда-ленными яичниками невозможна без применения препаратов эстрадиола, вызывающих пролифера-цию эндометрия и поддерживающих беременность на ранних сроках.За последние десятилетия накоплен огромный опыт назначения препаратов эстрогенов для подго-товки к беременности и поддержания ее на ранних сроках. В то же время ни в одной инструкции по при-менению препаратов эстрадиола, зарегистрирован-ных как в России, так и зарубежом, нет таких показа-ний, как подготовка эндометрия в программах ВРТ и поддержка лютеиновой фазы и ранних сроков бере-менности. Abstract:In vitro fertilization (IVF) has been utilized since the late 1970s. Since the time of the first successful pregnancy, use of IVF medication has changed over the years in regards to potency, timing, and types of medications given. Most commonly, these medications are given using a hypodermic needle, which causes some discomfort to the patient. Transdermal medications have several benefits, and this route could someday be utilized more frequently in IVF cycles. In this review, the current uses of transdermal medications in IVF are discussed, as well as future potential therapies.

Introduction: Management for poor response patients is still a challenge for clinician engaged in Assisted Reproductive Technology. Various protocols have been proposed to improve the outcome for patients, though no RCT can provide the evidence based of success rate of one protocol among others. Objective: Our purpose is to find the basic data of poor responder patients in Yasmin Clinic, Jakarta and to assess the IVF cycle outcome after the addition of recombinant Luteinizing Hormones. Material and methods: This is a cohort retrospective study taken from medical records of IVF patients at Yasmin Clinic, Cipto Mangunkusumo Hospital between January 2012 to January 2015. Result: Two hundreds and eighty-one poor response patients were selected for ART during January 2012 to January 2015, were divided into four main group. Control group received gonadotrophin protocol, while group II received additional recombinant Luteinizing hormones (rLH), Group III received additional Growth hormones (GH) and Group IV received additional rLH and GH both. Discussion: Our study demonstrated the follicle count, the oocyte count and the cleavage rate were statistically significant difference between control group and LH group. Where the oocyte count and follicle count were significantly higher, but the cleavage rate with LH showed negative correlation compare with control group. Conclusion: In this study we found that with adjuvant therapy with recombinant Luteinizing Hormones, produced higher follicles count and oocyte count.
