Skin metastases occur in 0.6%-10.4% of all patients with cancer and represent 2% of all skin tumors. Skin metastases from visceral malignancies are important for dermatologists and dermatopathologists because of their variable clinical appearance and presentation, frequent delay and failure in their diagnosis, relative proportion of different internal malignancies metastasizing to the skin, and impact on morbidity, prognosis, and treatment. Another factor to take into account is that cutaneous metastasis may be the first sign of clinically silent visceral cancer. The relative frequencies of metastatic skin disease tend to correlate with the frequency of the different types of primary cancer in each sex. Thus, women with skin metastases have the following distribution in decreasing order of frequency of primary malignancies: breast, ovary, oral cavity, lung, and large intestine. In men, the distribution is as follows: lung, large intestine, oral cavity, kidney, breast, esophagus, pancreas, stomach, and liver. A wide morphologic spectrum of clinical appearances has been described in cutaneous metastases. This variable clinical morphology included nodules, papules, plaques, tumors, and ulcers. From a histopathologic point of view, there are 4 main morphologic patterns of cutaneous metastases involving the dermis, namely, nodular, infiltrative, diffuse, and intravascular. Generally, cutaneous metastases herald a poor prognosis. The average survival time of patients with skin metastases is a few months. In this article, we review the clinicopathologic and immunohistochemical characteristics of cutaneous metastases from internal malignancies, classify the most common cutaneous metastases, and identify studies that may assist in diagnosing the origin of a cutaneous metastasis.

Introduction cutaneous adenoid cystic carcinoma is a rare epithelial tumor with slow development, it is most often described in the scalp; palpebral involvement is very rare. And is accompanied by local aggressiveness and frequent recurrence. The aim of this work is to discuss the management and the interest of a close surveillance of adenoid cystic carcinoma. Case presentation The authors report a case of palpebral cystic adenoid carcinoma in a 65-year-old patient who underwent surgical excision followed by reconstruction of the loss of substance. Discussion Cutaneous cystic adenoid carcinoma of the eyelids remains a very rare entity that must be evoked in front of a nodular cutaneous palpebral tumor that progresses slowly. An early diagnosis and a complete excision guarantee a better prognosis. Conclusion Adenoid cystic carcinoma, because of its local aggressiveness and its capacity to recur despite satisfactory surgical removal, requires long-term surveillance. Its surgery may be responsible for functional and aesthetic sequelae.

RESUMELes carcinomes adénoïdes kystiques (CAK) représentent 2 à 4% des tumeurs de la parotide et 12% des tumeurs malignes. Ce sont des tumeurs à croissance lente qui sont caractérisées par leur capacité à infiltrer les gaines nerveuses. Les auteurs rapportent trois cas de CAK de la glande parotide diagnostiqués et traités entre l'année 2000 et 2007. Il s'agissait de 2 hommes et une femme qui ont consulté devant l'apparition d'une tuméfaction de la loge parotidienne associée à une paralysie faciale dans un cas. Les aires ganglionnaires étaient libres. Tous les patients ont bénéficié d'une exploration comportant une échographie, une TDM dans un cas et une IRM chez un patient. Le traitement a consisté en une parotidectomie totale associée à un curage ganglionnaire dans tous les cas et à une radiothérapie complémentaire. L'évolution a été favorable dans deux cas. Le carcinome adénoïde kystique de la parotide est une tumeur rare, caractérisée par une évolution très lente, une agressivité locale, un caractère fortement récidivant localement et un haut pouvoir métastatique à distance. Mots-clés : Carcinome adénoïde kystique, glande parotide, chirurgie, radiothérapie. SUMMARYAdenoid cystic carcinoma (ACC) accounts for about 2 -4% of parotid tumours and 12% of parotid malignancies. It has a tendency for a prolonged clinical course, with nerve involvement. The authors report 3 cases of ACC arising in the parotid gland between 2000 and 2007. They were 2 men and a woman who consulted in front of the appearance of parotid masses associated with a facial paralysis in a case. We didn't find any cervical lymph nodes in the clinical exam. All the patients profited from an exploration comprising an ultrasonography, a TDM in a case and an IRM at a patient. All patients underwent surgery (total parotidectomy with neck dissection) and radiotherapy. The evolution was favorable in two cases. ACC of the parotid gland one is a rare tumour, characterized by a very slow evolution, a local aggressiveness, a character strongly repeating locally and a high metastatic capacity remotely.
