Whipple’s disease is an infectious, systemic and recurrent disease caused by the gram-positive bacterium Tropheryma whipplei. The disease proceeds with a heterogeneous clinical picture, presenting difficulties of timely diagnosis and in the absence of antibacterial therapy can lethal outcome. This review is devoted to the etiology, pathogenesis, epidemiology, clinical picture, modern diagnosis and therapy of Whipple’s disease.

Immunosuppressive Mechanisms in Brucellosis in Light of Chronic Bacterial Diseases

Brucellosis is considered one of the major zoonoses worldwide, constituting a critical livestock and human health concern with a huge socio-economic burden. Brucella genus, its etiologic agent, is composed of intracellular bacteria that have evolved a prodigious ability to elude and shape host immunity to establish chronic infection. Brucella’s intracellular lifestyle and pathogen-associated molecular patterns, such as its specific lipopolysaccharide (LPS), are key factors for hiding and hampering recognition by the immune system. Here, we will review the current knowledge of evading and immunosuppressive mechanisms elicited by Brucella species to persist stealthily in their hosts, such as those triggered by their LPS and cyclic β-1,2-d-glucan or involved in neutrophil and monocyte avoidance, antigen presentation impairment, the modulation of T cell responses and immunometabolism. Attractive strategies exploited by other successful chronic pathogenic bacteria, including Mycobacteria, Salmonella, and Chlamydia, will be also discussed, with a special emphasis on the mechanisms operating in brucellosis, such as granuloma formation, pyroptosis, and manipulation of type I and III IFNs, B cells, innate lymphoid cells, and host lipids. A better understanding of these stratagems is essential to fighting bacterial chronic infections and designing innovative treatments and vaccines.

Objetivo: Reconhecer as manifestações clínicas da Doença de Whipple (DW) assim como seu diagnóstico e manejo clínico. Revisão bibliográfica: A DW é uma infecção rara provocada pelo bacilo Tropheryma whipplei, que por se tratar de uma bactéria comensal, suspeita-se que o desenvolvimento está associado a sutis alterações genéticas do indivíduo. A progressão da DW é lenta e ocorre por anos, com uma sintomatologia inicial relacionada a perda de peso, artralgia e distúrbios intestinais. A sintomatologia inespecífica inicial é sobreposta por alterações cardiovasculares, hepáticas, neurológicas, cutâneas e reumatológicas graves nos estágios mais avançados. A biópsia do intestino delgado com a utilização do ácido periódico-Schiff é o principal método diagnóstico, devendo ser confirmada a presença do bacilo pela reação em cadeia da polimerase. O tratamento com antibióticos por um período prolongado tem mostrado bons resultados na redução dos sintomas e na erradicação da bactéria, podendo alcançar a cura. Considerações finais: A DW é uma infecção multissistêmica com diversas formas de apresentação clínica e de difícil diagnóstico devido a sua baixa prevalência. O reconhecimento da DW como possível causa das manifestações clínicas é fundamental para a solicitação de exames diagnósticos específicos para a detecção do bacilo, e assim iniciar o correto tratamento e acompanhamento.
