Current Applications of Diffusion Tensor Imaging and Tractography in Intracranial Tumor Resection

Abstract: In the treatment of brain tumors, surgical intervention remains a common and effective therapeutic option. Recent advances in neuroimaging have provided neurosurgeons with new tools to overcome the challenge of differentiating healthy tissue from tumor-infiltrated tissue, with the aim of increasing the likelihood of maximizing the extent of resection volume while minimizing injury to functionally important regions. Novel applications of diffusion tensor imaging (DTI), and DTI-derived tractography (DDT) have de… Show more

“…Over the last 15 years, white matter fiber tracking, using diffusion tensor imaging (DTI) has been investigated in different aspects of the surgical management of gliomas [8]. DTI is an in vivo neuroimaging technique that measures anisotropic water diffusion to extract white matter tractography [9].…”

“…9,10 DTI-merged functional neuronavigation could be a useful tool in brain tumor surgery for maximal safe resection. 11,12 Apart from the advantages associated with DTI, there are limitations concerned with DTI, such as lack of gold standard technique available to validate tractography and its reliability. The resolution of DTI may be insufficient which may lead to substantial partial volume effect.…”

“…There are several future works in development. We plan to conduct further multimodal image fusion studies based on the current research; for example, the integration of diffusion tensor imaging (DTI) into the 3D brain model created by FreeSurfer could lead to a more accurate judgment of the relationship between the tumor and surrounding nerve fiber bundles and help surgeons better protect white matter tracts (WMT) [22,23]. In addition, vascular (CTA/MRA, MRV) and skull (CT) models can be fused with the FreeSurfer models, and combined with Sina, these models could help surgeons make more accurate preoperative plans and yield more benefit for surgeon training and patient education.…”
