Current and future state of quality metrics and performance indicators in comprehensive medication management for ambulatory care pharmacy practice

Abstract: The American College of Clinical Pharmacy (ACCP) 2011 publication “Tenets for Developing Quality Measures for Ambulatory Clinical Pharmacy Services” describes comprehensive, accountable, feasible, scientifically sound, and usable quality metrics for ambulatory care (AC) practice. ACCP endorsed the definition of comprehensive medication management (CMM) in 2014 and has since advocated consistent implementation of CMM in patient‐centered, team‐based care. Given the decade of changes and advances in AC practice s… Show more

“…This observation echoes the findings of several meta‐analyses that have shown limited, if any, impact of pharmacist‐provided services on health outcomes, likely due to heterogeneity in the type of interventions and outcomes measured 78,79 . As a result, if pharmacists are to be consistently reimbursed as providers by third party payers, then, similar to past recommendations to encourage comprehensive medication management as a standardized approach, future efforts should focus on greater consistency in pharmacist‐led interventions as well as inclusion of comparator groups 70,80 …”

“…8 With the exception of the recommended CMM structure and process measure sets, validated measures specific for CMM, especially related to patient outcomes, have yet to be developed. 53 Although the outcomes measures recommended are useful for measuring discrete elements attributable to CMM services, they may fall short in comprehensively measuring the full effects of a CMM program. Thus, they may have reduced utility in assessing CMM value only to those who have the condition or use a particular medication.…”

“…It is important to acknowledge that the growth and development of pharmacist‐to‐pharmacist collaborative practices in these early stages contribute to the heterogeneity of outcomes. Clinicians and teams of collaborators can look to guidance documents/tools for proposed quality metrics to guide future design for alignment around key outcomes 46 …”
