Culture Proven Endogenous Bacterial Endophthalmitis in Apparently Healthy Individuals

Abstract: Though in most of the previously published literature endogenous endophthalmitis has been a metastatic ocular infection, the present study describes a series of endogenous bacterial endophthalmitis de novo in onset, without any identifiable predisposing factors. The overall age of presentation was in a younger population than in previously published series. The overall visual outcome was poor, probably due to the serious nature of disease itself and the relatively late presentation.

“…The most frequently seen pathogen was Staphylococcus aureus (6/19, accounted for 31.5%), followed by Streptococcus pneumoniae (3/19, 15.7%), and Pseudomonas aeruginosa (3/19, 15.7%) (Table 4). In other countries of South East Asia, the most common bacterium were Gram negative rods Klebsiella from hepatobiliary infections 7,19. In Europe and North America, the most common isolates were Gram positive cocci like Staphylococcus and Streptococcus from arthritis and endocarditis 8,19,21,22…”

“…Of note was the lack of substantial inflammation and better intraocular clarity in vancomycin-treated eyes, suggesting some beneficial level of anti-inflammatory activity. Vancomycin does not have anti-inflammatory activity per se, but can affect PMN phagocytic and chemotactic function 40,41 and inhibit proinflammatory cytokine responses in vitro 42,43 . If PMN recruitment or chemotaxis in the eye are affected by vancomycin, inflammation may be arrested during infection.…”

“…Underlying risk factors associated with an immunocompromised state are highly associated with cases of endogenous endophthalmitis, with only a limited number of the cases reported in otherwise healthy individuals [11,41,42]. The risk factors themselves are broad and range from underlying diabetes mellitus to infection with HIV [8,11,39,40,43].…”
