CT characterization of inflammatory paranasal sinus disease in cystic fibrosis

Abstract: The impaired mucociliary clearance in CF causes widespread inflammatory paranasal sinus disease, with inflammatory patterns more often requiring extensive surgery, with a higher risk of cerebrospinal fluid leak or bleeding, or involving areas that are more difficult to reach with the endoscope.

“…In 6 patients (32%), the mucocele caused partial obstruction, and in 13 patients (68%), the resulting obstruction was complete, often along with displacement of the nasal septum. This rate seems comparable to the 82% medial bulging of the lateral nasal wall in advanced maxillary disease among patients with CF who were not yet operated on, as described by Eggesbø et al 17 The presence of an anatomical obstruction, accompanied by subjective congestion or obstruction, is an accepted indication for considering surgery, 2,24 hence the importance of the imaging in these cases.…”

“…Normal SSCT is common in patients suspected of having sinusitis and has been reported in 27% [102]. In the normal population, only a few patients have more than one inflammatory pattern, while this phenomenon is common in patients with an underlying systemic disease as in CF patients [33].…”

“…Anatomical variants in the OMC may cause a predisposition to mucosal contact and obstruction of mucociliary clearance [27][28][29][30][31][32]. However, at imaging, pneumatisation variants are commonly displayed without concomitant obstruction of the drainage routes [30], and opacification may be seen without pneumatisation and anatomical variants [33,34]. Nasal septal deviation may interfere with mucociliary clearance due to mechanical obstruction of the OMC.…”

“…In four studies, 14,16,25,33 it was unclear whether the patients received antibiotics before or during sampling. Fifty percent of the studies 10,11,13,14,17,[22][23][24][26][27][28][29][30][31][32][33]36,38,39,42,45,47,49 provided information about culture methods. Most studies 10,11,14,17,[22][23][24][27][28][29][30][31][32]38,39,42,45,47,49 used conventional culture methods (41%).…”
