CT and MR Imaging Features of Primary Central Nervous System Lymphoma in Norway, 1989–2003

Abstract: BACKGROUND AND PURPOSE:Studying imaging findings of non-acquired immunodeficiency syndrome (AIDS) primary central nervous system lymphoma (PCNSL), we hypothesized that the imaging presentation has changed with the increasing incidence of PCNSL and is related to clinical factors (eg, time to diagnosis and the patient's being diagnosed alive or at postmortem examination).

“…These lesions are predominantly supratentorial (Koeller et al, 1997;Buhring et al, 2001). Ring like enhancement is rarely observed in immunocompetent patients, but can be commonly found in immunocompromised patients (Johnson et al, 1997;Haldorsen et al, 2008;2009;, Sutherland et al, 2012. In our study ring enhancement was observed in only two cases, one of which was labeled as Tuberculoma on radiological assessment.…”

“…In a previous study of non -AIDS associated PCNSL, 5% of patients had received immunosuppressive medications for medical conditions like ulcerative colitis, renal transplantation, rheumatoid arthritis and Wegener granulomatosis (Haldorsen et al, 2009).…”

“…2,3 Typical MR imaging features of PCNSLs are characterized by their periventricular locations, well-defined margin, moderate or marked edema, and intense and homogeneous nodular enhancement and are usually easy to correctly diagnose. [4][5][6] However, some patients present with atypical MR imaging features, commonly those of heterogeneous enhancement, such as patchy enhancement, streaky enhancement without mass formation, or even no enhancement. [6][7][8][9] Demyelinating diseases of the CNS are pathologic entities that are frequently encountered in clinical practice.…”

[6][7][8][9] Demyelinating diseases of the CNS are pathologic entities that are frequently encountered in clinical practice. When such lesions appear as solitary masses Ͼ2 cm in the longest diameter, they are defined as tumefactive demyelinating lesions (TDLs), and they can cause symptoms mimicking brain neoplasms and can be associated with variable enhancement on MR imaging.…”

“…Diffusion within the tumor is considered a surrogate marker of tumor cellularity, and water diffusion is often restricted in PLCNSLs because they are highly cellular tumors [11] . In our study, we did not observe a clear tendency in DWI, although in immunocompetent patients, twice as many lesions were restricted than in immunodeficient patients (perhaps due to the high percentage of necrosis).…”
